This report was released the NRC's [Nuclear Regulatory Commission] Special Inquiry Group to review and report on the nuclear accident at Three Mile Island, Pennsylvania. "The principal objectives of the inquiry were to determine what happened and why, to assess the actions of utility and NRC personnel before and during the accident and to identify deficiencies in the system and areas where further investigation might be warranted."
