  From the Document: "Long COVID [coronavirus disease] poses a conundrum for physicians and researchers alike. Representing a wide range of new, returning or ongoing health problems that arise about a month after initial infection, it can affect 20% to 30% of patients after recovery from even mild illnesses and about half of COVID-19 [coronavirus disease 2019] patients who required hospitalization. More than 200 symptoms associate with long COVID, but no universal clinical case definition exists. Long-haulers often don't know what to do about their lingering symptoms, which can include fatigue, shortness of breath, memory impediments, and gastrointestinal symptoms. Many different organ systems are involved, said Akiko Iwasaki, PhD, professor of immunobiology at Yale University and a principal investigator at the Howard Hughes Medical Institute."

• Understanding the Lived Experience of Long Covid: A Rapid Literature Review

  From the Introduction: "Long Covid (LC) has rapidly emerged as both a medical and social issue since 2020, drawing headlines in the public and global domain. In the UK, people experiencing ongoing symptoms of Covid after the initial infection came together online to try to make sense and gain recognition of their condition. Academic research into LC is constantly evolving and seeks to understand the illness from the perspective of those suffering from it. This rapid literature review explores existing studies into LC and particularly captures the narratives of people who have adapted to live with LC. The literature reviewed in this paper largely focuses on providing an initial understanding of LC, how the illness emerged, and the fluctuating symptoms managed by those with LC. Although this literature is predominantly descriptive and has a lack of focus on the ongoing trajectories and experiences of LC, narratives of making sense of, managing, and living with the illness over time are brought to the fore. Such narratives will be presented here by capturing the experiences and stories of those living with LC."

The Royal College of Speech and Language Therapists (RCSLT) undertook a survey of its members to inform our developing understanding of the clinical presentation of Long COVID and its impact on speech and language therapy services. Our findings strongly align with national data already collected on Long COVID, and highlight variation across the country with respect to the referrals received and services provided. The findings complement the emerging evidence base about the essential role of speech and language therapy in the assessment, intervention and management of Long COVID. We outline 11 recommendations to strengthen the role of speech and language therapy in Long COVID rehabilitation, to ensure that the needs of service users are fully met."

• Cargo Tank Trucks: Improved Incident Data and Regulatory Analysis Would Better Inform Decisions About Safety Risks, Report to Congressional Committees

  From the Highlights: "Cargo tank trucks deliver gasoline and other flammable liquids daily for consumer use. Trucks are loaded and unloaded through external bottom lines that, after loading, may contain up to 50 gallons of liquid and are known as 'wetlines.' Concerns have been raised about the safety of wetlines, since a collision may rupture them, releasing flammable liquid and possibly causing fatalities and property damage. PHMSA [Pipeline and Hazardous Materials Safety Administration] is responsible for regulating the safe transportation of hazardous materials and has proposed rules prohibiting the transport of flammable liquids in wetlines. In 2012, The Moving Ahead for Progress in the 21st Century Act required GAO [Government Accountability Office] to examine this issue. This report discusses (1) the extent that PHMSA's data reliably identify wetline safety risks, (2) options for addressing wetline safety risks, and (3) how well PHMSA has assessed the costs and benefits of addressing these risks through regulation. GAO analyzed PHMSA's wetline incident data for 1999 to 2011, reviewed PHMSA's regulatory cost-benefit analyses, and interviewed agency officials and industry and safety stakeholders. […] DOT [Department of Transportation] should improve its wetline incident data by requiring carriers to specifically report wetline incidents and by improving its information on incident consequences. DOT should also address uncertainty in the assumptions and data underlying its regulatory cost-benefit analysis. DOT did not agree or disagree with the recommendations, but provided technical comments."

  "H.R. 1003 would reauthorize the United States Advisory Commission on Public Diplomacy through fiscal year 2009. (The commission's authorization expired at the end of fiscal year 2006.) The commission makes recommendations to the Department of State on policies and programs related to public diplomacy, and may also appraise the effectiveness of such policies and programs. It reports to the Congress on its recommendations and appraisals. Based on information from the Department of State, CBO [Congressional Budget Office] estimates that implementing this legislation would cost about $300,000 a year over the 2008-2009 period, subject to the appropriation of the necessary amounts. Enacting the legislation would not affect direct spending or receipts."

Such transportation-disadvantaged populations may include those who are elderly, have disabilities, or have low incomes. Older adults represent the fastest-growing segment of the U.S. population, and access to transportation is critical to helping individuals remain independent as they age. This statement addresses (1) the federal programs that provide funding for transportation services for the transportation-disadvantaged populations, including older adults, and (2) the types of challenges faced in providing services to transportation-disadvantaged populations. This statement is based on GAO's [Government Accountability Office] body of work in this area from 2004 through 2012. GAO is not making any new recommendations. In 2012, GAO recommended that the Secretary of Transportation, as the chair of the Coordinating Council on Access and Mobility, along with its member agencies, should (1) complete and publish a strategic plan that would outline agency roles and responsibilities and articulate a strategy to help strengthen interagency collaboration and communication, and (2) report on the progress of Coordinating Council recommendations and develop a plan to address any outstanding recommendations.   From the Document: "The FAA [Federal Aviation Administration] Modernization and Reform Act of 2012 directed the Comptroller General of the United States to conduct a study on the effectiveness of the Federal Aviation Administration's (FAA) oversight of the use of new technologies to prevent or mitigate the effects of dense, continuous smoke in the cockpit of commercial aircraft. FAA oversees these procedures and technologies as part of its mission to provide the safest, most efficient aerospace system. In the initial phase of our research, we determined that there was only one technology that was developed to specifically target dense, continuous smoke in the cockpit and that this type of event occurred so infrequently that it was not practical for us to evaluate the effectiveness of FAA's oversight. However, we were able to identify relevant procedures that are more broadly targeted--for instance, at smoke, fire, or fume events. Consistent with the mandate, we gathered stakeholders' views regarding the effectiveness of FAA's oversight of these procedures and the technologies related to preventing or mitigating the effects of dense, continuous smoke in the cockpit."

Subsequently, the Act required FAA to work with industry to assess the certification process and address some of the findings in GAO's report. In July 2013, FAA issued reports on its efforts, including those in response to committee recommendations and FAA's implementation plans. This testimony addresses FAA's responses to the recommendations made by GAO in 2010 and the two joint FAA-industry committees concerning (1) the certification and approval processes and (2) the consistency of regulatory interpretation. It also discusses future challenges facing FAA's certification and approval processes. This statement is based in part on GAO's 2010 report. More detailed information on the objectives, scope, and methodology for that work can be found in that report.   From the Background: "This guideline has been developed collaboratively by NICE [National Institute for Health and Care Excellence], the Scottish Intercollegiate Guidelines Network (SIGN) and the Royal College of General Practitioners (RCGP). It covers care for people who have signs and symptoms that develop during or after an infection consistent with COVID-19 [coronavirus disease 2019], continue for more than 4 weeks and are not explained by an alternative diagnosis. This new and emerging condition, which has been described using a variety of terms including 'long COVID', can have a significant effect on people's quality of life. It also presents many challenges when trying to determine the best-practice standards of care based on the current evidence. There is no internationally agreed clinical definition or clear treatment pathway, and there is an evolving, evidence base. This guideline provides clinical definitions of the effects of COVID-19 at different times. It also provides advice on diagnosis and management based both on the best available evidence and the knowledge and experience of the expert panel."

The 9/11 Act included two key sections regarding our Nation's biosurveillance capabilities. Section 1101 authorized the National Biosurveillance Integration Center, or NBIC, and section 1102 requires the Government Accountability Office, GAO, to submit a report to Congress describing all Federal, State, and local biosurveillance efforts. Each of these will help protect against biological threats, whether man-made or natural. The key to stopping an outbreak from becoming an epidemic or an attack from becoming a catastrophe is early detection, identification, tracking, and response. Now, the National Biosurveillance Integration Center, NBIC, and the BioWatch program, housed in the Department of Homeland Security, are critical to this mission. Each of these programs are designed to provide early detection of disease outbreaks, a critical role in preventing or containing their spread. The NBIC was created to fulfill the requirements of Homeland Security Presidential Directive 9, 'Defense of U.S. Food and Agriculture,' issued in January 2004, or better known as HSPD--9." Millions of homeowners and businesses in Illinois and across the country, not to mention our recovering housing market, can ill afford the turmoil caused by a program lapse, which occurred during the previous Congress. […] There is no question that the program is in dire need of reform. For many years, the NFIP has been - for lack of a better phrase - underwater, with longstanding management and financial challenges, and was last reformed in 2004. The NFIP borrowed millions from taxpayers following the 2005 Gulf Coast hurricanes and continues to be financially unstable. Since 2006, the NFIP has been cited by the Government Accountability Office as a high-risk Federal program in need of fundamental reform. It is crucial that we work to restore the financial integrity of the NFIP so that homeowners and businesses in floodplain areas, like many in my State of Illinois, are not left without any protection, and taxpayers are not on the hook for the failure of the NFIP. We must work towards a long-term plan for flood insurance that eliminates taxpayer risk in the near-term. Important reforms to the NFIP must improve its financial stability, reduce the burden on taxpayers, and examine ways to increase private market participation." From the opening statement of Trey Gowdy: "The SAFE [Strengthen and Fortify Enforcement] Act was introduced to remedy this current unacceptable state of affairs. The bill, in my judgment, will keep us safe in numerous ways. First, it fulfills the intent of the Homeland Security Act of 2002, which authorized the placement of Department of Homeland Security Visa Security Units at highest-risk U.S. consular posts. This was an effort to address lapses in the current system, increase scrutiny of visa issuance, and prevent terrorists from gaining access to the United States. Unfortunately, since 2002 neither the State Department nor DHS has put a high priority on the establishment of Visa Security Units. Just recently, State Department denied DHS' request to set up a post in Turkey. Visa Security Units exist in only 14 countries. Meanwhile, close to 50 countries have been designated as highest risk. In addition to making it harder for terrorists to enter, the SAFE Act allows U.S. Officials to more easily remove terrorists and other national security threats. The bill closes loopholes and allows terrorists to be removed from American soil without threatening the disclosure of intelligence sources and methods." The only thing you need to spur than improve energy efficiency is profit-seeking companies responding rationally to high energy bills. Any company that doesn't use energy as wisely as possible will lose ground to a competitor that does. This is why free economies are the most efficient and have the lowest energy inputs per units of gross domestic product when you contrast that particularly with centrally-planned economies, which are certainly not as efficient." Rush: "Mr. Chairman, while today's hearing focuses on the progress made in the private sector, let us not forget that it was the leadership of State and Federal Government activities that paved the way for many of these energy efficiency successes. DOE [Department of Energy] rulemaking spurred dozens of national efficiency standards for appliances and equipment since 1987. ACCC-EEE [American Council for an Energy-Efficient Economy], rather, found that these existing standards will provide net savings of $1.1 trillion through 2035, while also reducing carbon pollution by the equivalent amount of taking approximately 118 coal-fired power plants offline by that same year. In fact, in 2010, overall U.S. energy use was 7 percent less than it would have been without these extending-existing, rather, standards."
