From the foreword of Deborah A.P. Hersman: "I am pleased to present the 2011 Annual Report to Congress for the National Transportation Safety Board (NTSB). [...] 2011 was a very productive year for the NTSB, and we are working as a team to advance our transportation safety mission with accountability, integrity and transparency. Demonstrating our commitment to accountability, we completed several major accidents reports within approximately one year of the accident date, which included a massive pipeline explosion in San Bruno, California, collision between a tour boat and barge in the Philadelphia, Pennsylvania harbor, and the airline collision in Aleknagik, Alaska. In addition to responding to accidents and issuing safety recommendations, we undertook activities to impart lessons learned from our investigations and raise awareness of emerging safety issues including distracted driving, airline safety and motorcoach safety. To that end, in 2011, we held public forums and conferences on airbags in general aviation accidents, family assistance, bus and truck safety, airline fuselage structural integrity, and public aircraft operations."

"The 2009 Annual Report presents a snapshot of the agency's work over the last year, providing details of completed and ongoing investigations conducted by our dedicated staff of investigative professionals. The Annual Report also showcases key achievements for the Most Wanted List of Transportation Safety Improvements. To provide more detail, clarity, and readability to the document, we have made several improvements to the 2009 Annual Report over last year. One example is the inclusion of regional and field location maps as well as accident investigation launch maps, which were added last year. Additionally, each section describes new significant achievements that illustrate how various organizational units of the NTSB contribute to the revised agency strategic objectives, which were published in the NTSB 2010-2015 Strategic Plan. Also, the agency 'At A Glance' box, which was introduced last year, now provides details on the status of our Most Wanted safety recommendation issue areas along with the number of accident investigation launches, report production statistics, the number of recommendations issued and closed, and staff demographic information. Furthermore, the Planning and Performance section now includes the Organizational Assessment, the overall performance report for the agency. We have updated the appendix highlighting the NTSB's progress in implementing Government Accountability Office recommendations, which includes GAO revised assessment of NTSB from October 2009. Finally, the 2009 Annual Report to Congress includes all new color photographs and numerous graphics, charts, and tables to help illustrate the scope and complexity of our work."

"The 2008 Annual Report presents a snapshot of the agency's work over the last year, providing details of completed and ongoing investigations conducted by our dedicated staff of investigative professionals. To provide more detail and clarity to the document, we have made a number of important improvements to the 2008 Annual Report over previous years. One example is the inclusion of maps identifying the locations of accident investigation launches to show the geographic breadth of our investigations. Additionally, each section describes 'significant achievements' that illustrate how various organizational units of the NTSB contribute to agency strategic objectives. Also, for quick review and evaluation of key statistical information, we have provided 'at a glance' boxes for each office showing, among other things, the number of accident investigation launches, report production statistics, the number of recommendations issued and closed, and staff demographic information. Furthermore, we have added new sections discussing the agency's accomplishments in International Outreach and Safety Activity, Information Technology, and Planning and Performance. We have also included an appendix highlighting the NTSB's progress in implementing Government Accountability Office recommendations. Finally, the 2008 Annual Report to Congress is printed in full color, including over 80 photographs and numerous other graphics."

The NTSB investigates accidents, conducts safety studies, evaluates the effectiveness of other government agencies' programs for preventing transportation accidents, and reviews the appeals of enforcement actions involving aviation and mariner certificates issued by the Federal Aviation Administration (FAA) and the U.S. Coast Guard, as well as the appeals of civil penalty actions taken by the FAA. To help prevent accidents, the NTSB develops safety recommendations based on its investigations and studies. These are issued to Federal, State, and local government agencies and to industry and other organizations in a position to improve transportation safety. Recommendations are the focal point of the NTSB's efforts to improve the safety of the nation's transportation system."

National Transportation Safety Board 2006 Annual Report to Congress
"To date, the NTSB [National Transportation Safety Board] has issued over 12,500 safety recommendations pertaining to the various transportation modes to more than 2,200 recipients. Because the Safety Board has no authority to regulate the transportation industry, its effectiveness depends on its reputation for conducting thorough and accurate investigations and for producing timely, well considered recommendations to enhance transportation safety. In 2006, the Safety Board continued to push for safety improvements as 89 recommendations were taken off its books after being implemented. This includes 30 aviation, 14 highway, 24 marine, 7 railroad, 10 pipeline and hazardous materials, and 4 intermodal safety advances recognized by the Board as meeting our recommendations in the past 12 months. They were officially closed 'acceptable action' or 'acceptable alternate action.' The overall acceptance rate for safety recommendations was about 82 percent."

"To date, the NTSB [National Transportation Safety Board] has issued about 12,350 safety recommendations pertaining to the various transportation modes to more than 2,200 recipients. Because the Safety Board has no authority to regulate the transportation industry, its effectiveness depends on its reputation for conducting thorough and accurate investigations and for producing timely, well-- considered recommendations to enhance transportation safety. In 2005, the Safety Board issued 84 safety recommendations and closed 142 recommendations, 111 of which were based on actions classified as 'acceptable.' In aviation, 29 were closed 'acceptable'; 37 in highway; 8 in pipeline and hazardous materials; 10 in marine, and 27 in rail. The Board also updated its Most Wanted list of vital safety recommendations targeted to Federal regulators and the States."
