  "On November 15, 1994, the Division of Safety Research (DSR) received a request from three fire fighters with the Valdosta Fire Department for a health hazard evaluation in investigating the circumstances of a hazardous materials incident, which occurred August 5, 1994, at the Lowndes Correctional Institute (LCI) in Valdosta, Georgia. The fire fighters requested technical assistance in reviewing both general and specific issues relating to this hazardous materials emergency response: proper incident command structure; training of personnel; the use of proper chemical protective clothing, including the provision of identically trained and equipped backup personnel during operations; medical evaluations of personnel, including baseline and routine periodic physicals and medical evaluations after exposure; and proper decontamination procedures."

  "On April 30, 1999, the Superintendent, City of Cincinnati Police Communication Section, contacted the National Institute for Occupational Safety and Health (NIOSH) to request a health hazard evaluation (HHE) at the Cincinnati Police Department's (CPD) 911 Communication Section (Call Center), located at 310 Ezzard Charles Drive, Cincinnati, Ohio. In addition, an HHE request was submitted by the President of American Federation of State, County, and Municipal Employees (AFSCME), Local 1543. Both management and union representatives were concern ed that work within the 911 Call Center was highly stressful, that workers were experiencing high psychological demands, potentially hostile interaction s with the public, difficulty overcoming distressing call-related memories, in creased tension and irritability, and chronic musculoskeletal discomfort because of improperly designed work stations. NIOSH investigators conducted a series of interviews with managers and employees at the CPD 911 Call Center. Following these meetings, NIOSH investigators developed a study protocol to evaluate each of the concerns expressed in t he HHE request and subsequently returned to the 911 Call Center to carry out the evaluation."

  "On November 12, 1996, the National Institute for Occupational Safety and Health (NIOSH) received a request from the International Association of Fire Fighters (IAFF) Local 416 on behalf of 78 fire fighters involved in a chemical warehouse fire that took place in Indianapolis, Indiana, on October 12, 1996. Fire fighters dispatched to the fire scene experienced chemical exposures and injuries. NIOSH was asked to provide technical assistance in reviewing the circumstances surrounding fire supression and overhaul activities, evaluate the overall incident command structure, and investigate chemical exposures and injuries experienced by fire fighters. Records were reviewed for 19 fire fighters who experienced acute health effects during and after fire suppression; these health effects included frequent headaches, chemical burns, irritation on face and hands, nose bleeds, cough, and a metallic taste in the mouth. Injuries experienced by two other fire fighters included a fractured pelvis and a concussion. This medical information was summarized in a letter dated May 23, 1997. Numerous reports and notes from the Indianapolis Fire Department's (IFD) Health and Safety Office, the Indianapolis Fire Investigation Unit, the local health department, and a local hospital were collected and later reviewed. Some fire fighters were interviewed (confidentially) in person and others interviewed by telephone to gather additional details of the incident. Deficiencies that contributed to injuries and illnesses were identified. Response activities, including medical care and monitoring, were reviewed to reconstruct events both during and after the fire incident."

  "On September 5, 1997, the National Institute for Occupational Safety and Health (NIOSH) received a request from fire fighters of the Racine Fire Department to conduct a health hazard evaluation (HHE) of diesel exhaust exposure at fire stations within the city of Racine, Wisconsin. Health concerns indicated on the request included headaches and concerns about the possible carcinogenic risk associated with exposure to diesel exhaust. In response to the request, NIOSH investigators conducted an industrial hygiene evaluation at two fire stations on November 12 and 13, 1997. Environmental monitoring was conducted for components of diesel exhaust including nitrogen oxide (NO), nitrogen dioxide (NO2 ), elemental carbon (EC), and miscellaneous hydrocarbons. Personal breathing zone (PBZ) and area air samples were collected for EC at each station. Area samples were collected in the kitchen, apparatus bay, and in the fire fighters' sleeping quarters. Additionally, one background EC sample was collected outside of each station as a comparative measure. Exhaust ventilation flow rates were also measured to estimate air changes per hour in apparatus bays."

  "In February 1992, the National Institute for Occupational Safety and Health (NIOSH) received a request for a health hazard evaluation (HHE) from a management representative of the Federal Bureau of Investigation (FBI) in Washington, D.C. The request was submitted because of concern about possible health hazards due to the exposure of fingerprint specialists to many of the chemicals that they use in processing latent fingerprints. NIOSH evaluated potential exposures to ethyl-2-cyanoacrylate, petroleum ether, iodine, acetone, titanium dioxide, and carbon black in the fingerprint laboratories and at a simulated crime scene. The laboratories were equipped with exhaust hoods, but there was no exhaust ventilation available at the simulated crime scene. NIOSH investigators also evaluated an argon laser that was used by fingerprint specialists to examine objects for visible fingerprints"

Each of 186 fire fighters from the Anchorage Fire Department were fit tested in 1992, while wearing a self- contained breathing apparatus (SCBA) (with nose cup) operated in the pressure-demand mode. The fit testing method used by Anchorage involved puffing irritant smoke from air flow indicator tubes into a test hood which encompassed the fire fighter's head and the SCBA's facepiece."

Pulmonary function tests (PFT) were also conducted on an equal number of fire fighters attending the symposium. The results of the PFT examinations were sent to the individual fire fighters who had participated in the screening, but further population analyses of the PFT data were not conducted, based on a determination by NIOSH medical personnel."

The IAFF reported that over 200 fire fighters and other response personnel were involved in the incident. [...] On the basis of the information obtained and reviewed during this investigation, the NIOSH investigators were able to identify several limitations that hampered the WFD's response to this high-rise incident. These limitations include the complexity of the building layout and the fact that the building was a high-rise, the lack of a formal ICS and appropriate pre-planning, communication problems during the incident and recall (general alarm) of WFD fire fighters, lack of coordination with fire fighters responding as part of mutual aid, fire fighter accountability, delegation of authority in areas such as incident command, safety, and logistics, confusion in both the incident command and staging area, and the use of SCBAs [self contained breathing apparatuses] by the fire fighters. This report summarizes work practices as they affected the health and safety of the fire fighters, including the Incident Command System (ICS) and safety management. This report also discusses decontamination of fire fighter personal protective equipment and provides recommendations based on the findings of this investigation."

  The purpose of this document is to provide information and guidance that will help "[t]o protect workers from illnesses and injuries associated with livestock and poultry wastewater and sludge from animal feeding operations during and after floods. [...] Farms, animal production sites, and processing plants contain animal waste. Waste or manure can be located in places such as animal housing, open lots, pastures, manure treated fields, sediment basins, above ground storage structures, underground storage tanks, lagoons, storage ponds, and manure drying or composting sites. Workers may be at risk of exposure to animal diseases that may be passed from animals through contact with workers' broken skin, eyes, nose, mouth, or mucous membranes. Diseases may spread from contact with animals' infected body fluids or tissues and from contaminated water or materials. Manure and wastewater may also contain additional hazardous compounds such as chemicals, hormones, antibiotics, metals, and allergens. In addition workers may encounter musculoskeletal hazards (e.g., lifting heavy animals and objects), physical hazards (e.g., unsafe buildings and structures, sharp objects, heat, noise), chemical hazards (e.g., fuels, fertilizers, pesticides, cleaning agents, hydrogen sulfide) and allergens (e.g., latex and animal saliva, skin, urine)."

[...] Researchers have not found exactly how much exposure to dampness-related substances it takes to cause health problems. Research studies report that finding and correcting sources of dampness is a more effective way to prevent health problems than counting indoor microbes.   "The health of those who live, attend school, or work in damp buildings has been a growing concern through the years due to a broad range of reported building-related symptoms and illnesses. [...] Researchers have not found exactly how much exposure to dampness-related substances it takes to cause health problems. Research studies report that finding and correcting sources of dampness is a more effective way to prevent health problems than counting indoor microbes. Therefore, NIOSH [National Institute for Occupational Safety and Health] developed a tool to help assess areas of dampness in buildings and to help prioritize remediation of problems areas."

• Evaluation of Law Enforcement Officers' Potential Occupational Exposure to Illicit Drugs -- Virginia

  From the Document: "In April 2018, a police department requested a health hazard evaluation (HHE) concerning unintentional potential exposure to illicit drugs among police officers during a response to a 911 call. The call was about a person who reportedly had a drug overdose in a hotel room. Four police officers developed adverse health effects during the incident. [...] Recommendation 1: Continue periodic training to employees on how to prevent occupational exposure to illicit drugs. Training topics include standard safe operating procedures, PPE, and decontamination. [...] Recommendation 2: Develop departmental policies and procedures that are more specific for police work involving opioids (and other illicit drugs). [...] Recommendation 3: Working with 911 dispatch coordinators, identify possible improvements in information gathering and communication before police officers arrive at scenes where there might be illicit drugs. [...] Recommendation 4: Encourage officers to report possible exposures to and health effects resulting from exposure to illicit drugs to their supervisors."

  "The Centers for Disease Control and Prevention (CDC) and Agency for Toxic Substances and Disease Registry (ATSDR) are working with federal, state and local agencies as well as global health partners in response to Hurricanes Harvey, Irma, and Maria. Protecting the health and safety of responders and volunteers by preventing diseases, injuries, and fatalities is a National Institute for Occupational Safety and Health (NIOSH) Emergency Preparedness and Response Program priority. This document focuses on key messages for employers, workers, and volunteers responding to Hurricanes Harvey, Irma, and Maria. It will be updated as new information becomes available and will be distributed regularly. Please share this document with others as appropriate."
