Biological Disease Outbreak - Pandemic Influenza: Scenario
"An outbreak of unusually severe respiratory illness is occurring in China. The US Centers for Disease Control (CDC) has identified the particular strain as Type A H7N3, a subtype never before isolated from humans. The CDC also reports that the H7N3 virus has been isolated from ill airline passengers and large numbers of cases are now being reported in Hong Kong, Singapore, South Korea, Japan and the United States. Young adults are most severely affected and case-fatality rates approach 50%. State and local agencies are asked to intensify influenza surveillance and implement airborne protection measures for staff. News agencies have issued alerts for anyone experiencing flu-like symptoms to immediately contact their health care providers. There is an increase in the number of persons presenting to emergency rooms with symptoms consistent with influenza. More people are seeking medical care than actually need it. Personnel in key positions are absent due to illness, fear of illness or caring for ill family members. Local pharmacies have run out of antiviral medications and are unsure whether they can expect to receive more. Estimates indicate that 10% of the population is ill with H7N3 influenza. Local hospitals and outpatient clinics are extremely short-staffed; an estimated 30- 40% of physicians, nurses and other healthcare workers are absent. Intensive care units are overwhelmed, and there is a shortage of mechanical ventilators for patients with severe respiratory syndromes or postoperative needs. Family members are distraught and outraged when loved ones die within a matter of days. All essential services have personnel shortages, resulting in major reductions in routine services. There are shortages of food supplies due to
the nationwide impact."

Department of Defense Implementation Plan for Pandemic Influenza
"This plan implements the White House 'National Pandemic Influenza Strategy Implementation Plan.' It provides strategic guidance to all Department of Defense components for preparation and response to an outbreak of pandemic influenza. The Secretary of Defense's principal responsibility in responding to a pandemic will be to protect U.S. interests at home and abroad. This implementation plan sets forth Department of Defense (DoD) guidance and addresses key policy issues for pandemic influenza planning. This guidance will enable the Combatant Commanders, Military Departments, and DoD agencies to develop plans to prepare for, detect, respond to, and contain the effects of a pandemic on military forces, DoD civilians, DoD contractors, dependents, and beneficiaries. Additionally, plans will address the provision of DoD assistance to civil authorities both foreign and domestic. Finally, attention to the key security concerns, such as humanitarian relief and stabilization operations that may arise as a result of a pandemic, will be addressed."

Philadelphia Department of Public Health: Pandemic Influenza Preparedness Plan
The Pandemic Influenza Preparedness Plan is an Annex to the Philadelphia, Public Health Emergency Response Plan, which supplements the City's overall, Emergency Operations Plan. The Pandemic Influenza Plan identifies the, cooperating City agencies involved in preparedness for pandemic flu and their, respective response roles. The Pandemic Influenza Preparedness Plan also describes how the Philadelphia Department of Public Health (PDPH) will work with local partners, such as physicians, hospitals, and community agencies, to respond to an outbreak of disease due to a novel strain of influenza. Avian influenza (H5N1) is just one such novel strain.

"The following questions and answers will (1) assist Federal agencies in understanding and using policies and programs designed to accomplish work during a pandemic health crisis and (2) assist Federal employees and their families in dealing with the consequences of a pandemic health crisis. Employees should consult with their agency human resources office regarding these policies and programs, since many of these matters are addressed in agency internal policies and/or collective bargaining agreements. In addition, agencies will provide guidance and specific direction to their employees on the agency's operating status, including whether employees should work from an alternative worksite for a period of time. Please note that some of the questions posed and answered in this guidance are hypothetical in nature and do not reflect actual events or circumstances that have occurred (such questions are clearly marked as '[hypothetical]')."

New York City Department of Health and Mental Hygiene Pandemic Influenza Preparedness and Response Plan
"The most densely populated city in the United States and a major international port of entry, New York City (NYC) has long been vulnerable to infectious disease threats, whether naturally occurring or intentional. In the event a novel influenza strain develops the capacity for efficient human-to-human transmission anywhere in the world, it would not take long to reach NYC. Such an outbreak poses wide-ranging challenges, including the potential for huge numbers of illnesses and deaths, a severely strained health care system, and difficult psychosocial consequences for a large proportion of the population, especially the homeless, the homebound, and other vulnerable New Yorkers. Aware of our susceptibility, the New York City Department of Health and Mental Hygiene (NYC DOHMH) has for many years placed great emphasis on emergency preparedness planning, even before 9/11. Working closely with our colleagues in the emergency services, health care, business, and not-for-profit sectors, DOHMH continues to engage in extensive preparedness efforts-more than 50 tabletop, functional, and full-scale exercises in the past 5 years-to test, assess, and strengthen our response capabilities. The NYC DOHMH Pandemic Influenza Preparedness and Response Plan draws on lessons learned from these efforts, providing a comprehensive, scalable, and flexible strategy to protect the health of New Yorkers."
