  "The Food and Drug Administration (FDA) is issuing this guidance to inform industry and FDA staff of the Agency's recommendations for managing postmarket cybersecurity vulnerabilities for marketed and distributed medical devices. In addition to the specific recommendations contained in this guidance, manufacturers are encouraged to address cybersecurity throughout the product lifecycle, including during the design, development, production, distribution, deployment and maintenance of the device. A growing number of medical devices are designed to be networked to facilitate patient care. Networked medical devices, like other networked computer systems, incorporate software that may be vulnerable to cybersecurity threats. The exploitation of vulnerabilities may represent a risk to health and typically requires continual maintenance throughout the product life cycle to assure an adequate degree of protection against such exploits. Proactively addressing cybersecurity risks in medical devices reduces the overall risk to health."

  "The first document is a summary by state of legal and regulatory requirements related to the smart grid. The document contains three US maps that show how activities in the states are progressing. (See the right hand column.) The report contains a summary of legislative and regulatory activity in each state and also contains links to the resource material used to construct the summaries. These state smart grid‐related activities are centered on smart meters (Advanced Metering Infrastructure), data privacy issues, opt out policies and regulations promoting net metering and distributed generation programs. In addition, associated programs are discussed, such as energy efficiency goals and requirements, dynamic pricing policies and demand response programs. For many states, the document also includes a discussion of key drivers of increased activity. The second document provides 23 case studies of smart grid pilots and programs in the U.S.  It provides a sample of projects chosen to provide a variety of smart grid applications and approaches. They involve various technology types, pricing programs, and funding mechanisms.   Each case study begins with a table that outlines important elements of the project and presents quantitative results and metrics when available.  The tables are followed by more detailed descriptions of the projects, along with further supporting information."

  From the executive summary: "The 2012 Trafficking In Persons Symposium (Salt Lake City, April 10-13) sponsored by the Department of Justice, Office of Juvenile Justice and Delinquency Prevention, examined domestic child sex and labor trafficking to glean, compile, and disseminate first-hand knowledge and experience from 127 subject matter experts (SMEs) brought together from law enforcement, the judicial system, social services, education, health care and other community and faith-based initiatives. […] Using surveys, focus groups, and interviews, Symposium attendees generated a wealth of new and experience-based information relating to not only what is currently happening in the crime of child trafficking in this hemisphere, but also tested and best practices relating to child trafficking identification, interviewing and accessing treatment for victims, investigation and prosecution of traffickers and survivor perspectives on trafficking effects and dynamics. The Trafficking in Persons Symposium's prime objective was to use this information to develop SME informed training for first responders, child welfare and juvenile justice personnel, medical professionals, educators, social service providers and the public-aimed at supporting and enhancing abilities to identify and respond to child trafficking. The surveys before the Symposium and the focus groups during the Symposium examined five primary issues: (1) identification of victims and traffickers, (2) engaging victims, (3) investigation and prosecution of traffickers, (4) response and restoration of victim/survivors, and (5) the survivor perspective; and the study results are discussed on the following pages."

• Preventing Deaths and Injuries of Fire Fighters During Training Exercises

  "The role of the fire department has evolved over the years to encompass more than traditional fire suppression. Fire fighters respond to hazardous material incidents, combustible-metal fires, large-scale disasters, and terrorist events. They perform trench, high incline, and confined space rescues and provide emergency medical service calls. Even fire suppression has become multifaceted with changes in technology and the fireground environment. In addition, residential home fires have decreased approximately 50% over the past 40 years. The expanded role and decrease in structure fire fighting experience of the fire fighter has made training a necessity for carrying out the mission of the fire service as safely as possible. [...] Inadequate training and training without safeguards can be life threatening to fire fighters. National consensus standards for training programs have been established for decades; however, some fire departments are not conducting training in accordance with these established standards. The reasons can include budgetary constraints, time constraints, and/or a lack of understanding the standards and the benefits of complying with them. Legal precedent has established that a lack of understanding of the standards will not absolve the department of responsibility and accountability for a fire fighter injury or death during training."

  "Advances in emerging surveillance technologies like cell-site simulators - devices which transform a cell phone into a real-time tracking device - require careful evaluation to ensure their use is consistent with the protections afforded under the First and Fourth Amendments to the U.S. Constitution. The United States' military and intelligence agencies have developed robust and sophisticated surveillance technologies for deployment in defense against threats from foreign actors. These technologies are essential to keeping America safe. Increasingly though, domestic law enforcement at the federal, state, and local levels are using surveillance technologies in their every-day crime-fighting activities. In the case of cell-sitesimulators, this technology is being used to investigate a wide range of criminal activity, from human trafficking to narcotics trafficking, as well as kidnapping, and to assist in the apprehension of dangerous and violent fugitives. Law enforcement officers at all levels perform an incredibly difficult and important job and deserve our thanks and appreciation. While law enforcement agencies should be able to utilize technology as a tool to help officers be safe and accomplish their missions, absent proper oversight and safeguards, the domestic use of cell-site simulators may well infringe upon the constitutional rights of citizens to be free from unreasonable searches and seizures, as well as the right to free association. Transparency and accountability are therefore critical to ensuring that when domestic law enforcement decide to use these devices on American citizens, the devices are used in a manner that meets the requirements and protections of the Constitution."

  "IHS [Indian Health Service] is charged with providing health care to American Indian/Alaska Native (AI/AN) people who are members or descendants of 567 tribes. AI/AN people born today have a life expectancy that is 4.4 years lower than all races in the United States, and they continue to die at higher rates than other Americans from preventable causes. Concerns about the quality of care provided to AI/ANs in IHS facilities have been identified recently by federal officials and tribal members. GAO [Government Accountability Office] was asked to review how IHS oversees the quality of care provided in its facilities. This report examines IHS's oversight of the quality of care provided in its federally operated facilities. GAO reviewed policies and guidance related to quality of care in federally operated facilities and interviewed IHS officials at the headquarters level and all nine area offices with federally operated facilities. GAO also examined documents from governance meetings between area office and facility staff. GAO recommends that the Secretary of the Department of Health and Human Services direct the Director of IHS to (1) as it implements its quality framework, ensure that agency-wide standards for the quality of care provided in its federally operated facilities are developed, that facility performance in meeting these standards is monitored over time, and that enhancements are made to its adverse event reporting system, and (2) develop contingency and succession plans for the replacement of key personnel.   "GAO [Government Accountability Office] has reported that DOD has not collected uniform data on sexual assaults involving members of the armed forces. In 2008, Congress required DOD to implement a centralized, case-level database for the collection and maintenance of these data. In 2012, DSAID [Defense Sexual Assault Incident Database] reached initial operational capability to capture sexual assault data. House Report 112-479 included a provision for GAO to review DSAID no sooner than 1 year after it was certified compliant with DOD standards by the Secretary of Defense. This report (1) describes the current status of DOD's implementation of DSAID and steps DOD has taken to help standardize DSAID's use, (2) assesses any technical challenges DSAID's users have identified and any DOD plans to address those challenges, and (3) assesses the extent to which DOD's change management process for modifying DSAID aligns with information technology and project management industry standards. GAO reviewed DOD documents, and interviewed DOD program officials as well as DSAID users. Specifically, GAO conducted site visits to 9 military installations and met with 42 DSAID users. Views obtained are nongeneralizable. Installations were selected based on their use of DSAID, number of users, geographic diversity, and other factors. GAO is not making recommendations in this report. DOD provided technical comments, which GAO incorporated as appropriate."

• Public Health Emergencies: HHS Needs to Better Communicate Requirements and Revise Plans for Assessing Impact of Personnel Reassignment, Report to Congressional Committees

  "HHS [U.S. Department of Health and Human Services] provides funding to state, local, and territorial entities to help them prepare for and respond to public health emergencies, such as influenza pandemics and other threats. However, states have reported not having sufficient personnel to assist in public health emergencies. The Pandemic and All-Hazards Preparedness Reauthorization Act of 2013 (PAHPRA) authorized HHS to allow states and tribes to temporarily reassign personnel funded, in whole or in part, by HHS programs to aid in public health emergency response. PAHPRA included a provision for GAO [Government Accountability Office] to examine the impact of the reassignment authority. However, the authority has not yet been used. Therefore, this report examines the processes HHS has in place to review states' and tribes' requests for temporary reassignment and evaluate the after-action reports states and tribes are to submit after they have used the authority. GAO reviewed HHS guidance and interviewed officials from HHS and five states selected based on their increased risks for public health emergencies and levels of prior federal grant funding received. GAO recommends that HHS direct ASPR [Office of the Assistant Secretary for Preparedness and Response] to (1) conduct outreach to HHS agencies and offices to inform them of ASPR's processes, expectations, and requirements for the reassignment authority; and (2) develop a plan to evaluate after-action reports to assess the authority's impact on emergency response and medical surge. HHS agreed with both recommendations and provided information on how ASPR plans to implement them."

  From the Executive Summary: "CSRIC [Communications Security, Reliability and Interoperability Council] IV Working Group 4 (WG4) was given the task of developing 'voluntary mechanisms' that give the Federal Communications Commission (FCC) and the public assurance that communication providers are taking the necessary measures to manage cybersecurity risks across the enterprise.  WG4 also was charged with providing implementation guidance to help communication providers use and adapt the voluntary NIST Cybersecurity Framework (hereinafter 'NIST CSF'). [...] By building on the cross‐sector NIST CSF and by framing its applicability to five major communications industry segments, the Working Group was able to formulate and commit to several voluntary mechanisms that provide the macro‐level assurances sought by the FCC.  Moreover, these mechanisms, combined with the insights, tools, guidance, and fact‐based analyses developed by over 100 cybersecurity professionals who participated in a year‐long effort to produce this report, validate the advantages of a non‐regulatory approach over a prescriptive and static compliance regime."

  "In the era of continuous compromises, business executives are faced with the growing threat of malicious actors targeting and successfully penetrating their defenses at an alarming rate. Cyber-attacks are making the news daily, especially when they target major retailers, banks, and government agencies. Increasingly, businesses are relying on the connectivity (always online) and productivity (customization/remote access) of their information systems. Businesses are challenged with trying to deliver services to an expanding consumer base while trying to balance security with the convenience of usability. Connectivity, productivity and convenience are critical in the 21st century, but if not implemented properly, they can have devastating effects to security. Striking a balance between security and usability is not easy and is a constantly evolving process."

• Safety Study: Supervisory Control and Data Acquisition (SCADA) in Liquid Pipelines

  "In the pipeline industry, Supervisory Control and Data Acquisition (SCADA) systems are used to collect data from pipeline sensors in real time and display these data to humans who monitor the data from remote sites and remotely operate pipeline control equipment. This National Transportation Safety Board study was designed to examine how pipeline companies use SCADA systems to monitor and record operating data and to evaluate the role of SCADA systems in leak detection. The number of hazardous liquid accidents investigated by the Safety Board in which leaks went undetected after indications of a leak on the SCADA interface was the impetus for this study. [...] As a result of this study, the Safety Board issued five recommendations to the Pipeline and Hazardous Materials Safety Administration."
