Coast Guard Health Records: Timely Acquisition of New System is Critical to Overcoming Challenges with Paper Process, Report to Congressional Requesters
"In 2010, the Coast Guard initiated an effort--known as IHiS [Integrated Health Information System]--to replace its aging EHR [electronic health record] system with a suite of modernized systems that was to automate various health care services for its nearly 50,000 military members. However, in October 2015, the Coast Guard announced that the modernization project would be canceled. GAO [Government Accountability Office] was asked to review the Coast Guard's efforts to develop a modernized EHR system. GAO's objectives were to (1) describe what led the Coast Guard to terminate further IHiS development, and how much was spent on the project; (2) evaluate the Coast Guard's management and oversight for the discontinued project and what, if any, lessons learned were identified; (3) describe the Coast Guard's current process for managing health records and the challenges, if any, it is encountering; and (4) determine the Coast Guard's plans for effectively implementing a new EHR system and the current status of its efforts."

"H.R. 3851 would explicitly authorize the Department of State to offer awards under its War Crimes Rewards Program for crimes that would be prosecuted under the laws and statutes of the United States and other countries. Currently, under that program, the department uses appropriated funds to offer cash awards of up to $5 million for information leading to the arrest or conviction of designated individuals accused by international tribunals of committing war crimes, genocide, or crimes against humanity."

Nuclear Weapons: NNSA Should Adopt Additional Best Practices to Better Manage Risk for Life Extension Programs, Report to the Subcommittee on Strategic Forces, Committee on Armed Services, U.S. Senate
"Weapons in the U.S. nuclear stockpile are aging. NNSA [National Nuclear Security Administration] and the Department of Defense undertake LEPs [life
extension programs] to refurbish or replace nuclear weapons' aging components. Prior LEPs experienced cost overruns, schedule delays, and scope reductions, and prior GAO [Government Accountability Office] reports identified the need for NNSA to use EVM [earned value management] and conduct TRAs [technology readiness assessments] to address program risks. In 2013, NNSA developed a management approach for LEPs that it regards as an improvement and currently manages three LEPs using its revised approach. NNSA and its contractors conduct the work associated with these LEPs at seven sites across the country. GAO was asked to review NNSA's management of its LEPs using its revised approach. This report assesses the extent to which NNSA has implemented, consistent with best practices, the use of EVM and TRAs in its management of LEPs."

Tax Fraud and Noncompliance: IRS Can Strengthen Pre-Refund Verification and Explore More Uses, Report to Congressional Requesters
"Beginning in 2017, as required by law, the Internal Revenue Service (IRS) held all refunds for taxpayers claiming the Earned Income Tax Credit (EITC) or Additional Child Tax Credit (ACTC) until February 15. IRS also took actions to verify wage and other information reported on tax returns before issuing refunds, referred to as systemic verification, but several factors limited its success. IRS received over twice as many (over 214 million) Forms W-2, Wage and Tax Statement (W-2) by February 15 compared to the same time in 2016, and reported that W-2 data were responsible for improving fraud detection and reducing taxpayer burden. [...] IRS continues to confront the ongoing problems of identity theft (IDT) refund fraud. The agency estimates that at least $1.68 billion was paid in IDT refund fraud in 2016. To help address this issue, consistent with GAO's [Government Accountability Office] prior reporting, the Protecting Americans from Tax Hikes Act of 2015 advanced the deadline for employers to file W-2s to SSA [Social Security Administration] to January 31 (about 1 to 2 months earlier than in prior years). This change allows IRS more time to match wage information to tax returns through systemic verification, and identify any discrepancies before issuing refunds. GAO was asked to assess how well IRS implemented systemic verification."

Nationwide Public Safety Broadband Network: First Steps
"February 22, 2012 will long be remembered as a landmark day for public safety and emergency communications. It was on that day that President Barack Obama signed into law the Middle Class Tax Relief and Job Creation Act of 2012 (H.R.3630). The signing of that Act was a significant win for the public safety community nationwide. It was the culmination of over a decade of advocacy to see the reallocation of the 'D Block' spectrum to public safety and to fulfill one of the last outstanding recommendations of the 9/11 Commission: the development of a nationwide interoperable communications network. In addition to the reallocation of the D Block to public safety, the Act provides authorization and funding to establish the Nationwide Public Safety Broadband Network (NPSBN). This includes $7 billion to build the network, including funding for research and development, a nationwide governance structure to oversee the network, and access to the network for Federal, State, and local public safety and secondary users such as utility companies, transportation, and other critical infrastructure providers. The development and deployment of a Nationwide Network will be an accomplishment that will significantly advance the Nation's public safety communications capabilities, and enhance the ability of first responders across the country to provide life-saving services. But this network will not be developed overnight. There remains a great amount of work to be done at every level of government to make this new network become reality."

Combating the Opioid Crisis
"The effects are disastrous and heartbreaking. The reach is from coast to coast. The opioid crisis has truly reached epidemic proportions, fueled by dangerous new chemical versions and illicitly manufactured synthetic drugs. Scan the headlines on any given day and you'll hear about a life gone too soon to addiction or about a raid that seized obscene quantities of prescription painkillers or illicit drugs. It is killing our friends, family members and neighbors in communities across the country. The opioid epidemic is a deep and multi-faceted issue that we have examined through numerous aspects of our work at the House Energy and Commerce Committee. From the earliest hearings before our Oversight and Investigations subcommittee to legislative solutions tested in our Health subcommittee, our multi-year, multi-Congress findings have led to bills that are now law - namely the Comprehensive Addiction and Recovery Act and the 21st Century Cures Act. But just as the scourge continues across every county in the United States, we must redouble our efforts to match the growing crisis. Those actions range from ensuring successful implementation of the laws passed last year to continuing to investigate this crisis and identify additional solutions that will help those battling addiction and those in law enforcement and the health care professionals responding to the crisis."

"To think through the strategically impactful events that might come in 2018--precisely because, after the devastation on September 11, 2001, the great criticism of the national security enterprise was that 'the most important failure was one of imagination.' This rebuke hurt even more because a plane-borne attack had been predicted, not by our own government, but in a Tom Clancy novel. The list below--the flock of potential, future-shaking events to come--is every bit as speculative as a novel's plotline. So nearly all these will be wrong. But to look upon a high 'failure' rate in such an endeavor would miss the point. The proper objective with such an exercise is to make many big bets that have highly lucrative upsides and massive returns on investment (like a venture capitalist's investment strategy). That way, getting 10 or 5 or even 1 percent 'right' leads to outsize yields and incredible net gains. In the case of this exercise, if even one of these turns out to be close to correct, then the advance consideration will have been worth it."

The bill would authorize the appropriation of $735 million over the 2018-2022 period for the United States Geological Survey (USGS), the National Science Foundation (NSF), the Federal Emergency Management Agency (FEMA), and the National Institute of Standards and Technology (NIST) to continue the program."

State Progress in Record Reporting for Firearm-Related Background Checks: Fugitives from Justice
"Under the Brady Handgun Violence Prevention Act of 1993 (Brady Act), being a fugitive from justice prohibits a person from possessing or purchasing firearms. 'The Code of Federal Regulations' defines a 'fugitive from justice' as any person who has fled from any state to avoid prosecution for a felony or a misdemeanor; or any person who leaves the state to avoid giving testimony in any criminal proceeding. The term also includes any person who knows that misdemeanor or felony charges are pending against such person and who leaves the state of prosecution. Following passage of the Brady Act, the Department of Justice provided guidance for applying this prohibition. Accordingly, individuals with qualifying or prohibiting outstanding warrants in any state or federal database searched as a part of a firearms background check are ineligible to receive firearms from Federal Firearms Licensees (FFLs). […] Given the definition of fugitive from justice-coupled with how the NICS Section and Point-of-Contact5 (POC) states apply the definition to determine a person's eligibility to possess or purchase a firearm- the importance of making warrant information available to databases checked by NICS cannot be overemphasized. This bulletin describes the requirements for entering warrants into databases used by NICS and explains the challenges of entering warrant records into state and national databases. Finally, it describes recent progress made by states in making warrant records more readily available to the NICS."

America's Airports: The Threat From Within, House Homeland Security Committee Majority Staff Report
"Approximately 900,000 people work at these airports, and many are able to bypass traditional screening requirements that travelers visiting the airports must endure. While the overwhelming majority of these airport workers take the inherent responsibility seriously, there are increasing concerns that insider threats to aviation security are on the rise. Recent insider threat examples discussed in this report include an attempt to detonate a bomb at an airport, gun and drug smuggling, an expressed willingness to smuggle explosives as well as employees who became involved in terrorist activities overseas. In all of these instances, the employees in question had access to secure areas of the airport. These insider threats, and the lack of adequate access controls at airports nationwide, are of particular concern given the rise of terrorist groups bent on penetrating U.S. airport security to commit terrorist acts and 'lone wolf' attacks being inspired by terrorist groups like ISIS [Islamic State of Iraq and Syria."

Medical Devices: Cancer Risk Led FDA to Warn Against Certain Uses of Power Morcellators and Recommend New Labeling, Report to Congressional Requesters
"In December 2013, media reports raised concerns regarding the use of power morcellators in the surgical treatment of women with uterine fibroids. These concerns focused on the spread of an unsuspected uterine cancer after such use of the devices. GAO [Government Accountability Office] was asked to review power morcellator medical devices. This report examines (1) the number of 510(k) submissions for power morcellators FDA [Food and Drug Administration] cleared, and the extent to which the agency determined the devices had new intended uses or new technological characteristics; (2) FDA's understanding of any concerns with the use of power morcellators to treat uterine fibroids prior to receiving adverse event reports, and the actions FDA has taken in response to these reports; and (3) the professional standards and guidance for physicians regarding the use of power morcellators, and the information device manufacturers provided. GAO reviewed documentation of FDA's decision-making and guidance and manufacturers' device labeling, and interviewed FDA officials. In addition, GAO reviewed documents and contacted officials from 10 professional societies and other organizations that have a potential interest in the use of power morcellators, and three health care providers that performed gynecological procedures that could involve the use of the devices. GAO also contacted all 12 manufacturers for the power morcellators FDA cleared for the U.S. market. The Department of Health and Human Services provided technical comments on a draft of this report, which were incorporated as appropriate."

From the document: "A properly regulated system of legal immigration is in the national interest of the United States. Such a system enhances the benefits of immigration while protecting against potential harms. Immigrants often create new businesses and other employment generating activities that promote the renewal of city neighborhoods and commercial districts. Immigrants also can strengthen America's economic and political ties with other nations and, thus, enhance our ability to compete in a global economy and provide leadership in international and humanitarian affairs. Properly regulated immigration further strengthens American scientific, literary, artistic and other cultural resources. It promotes family values and ties, important components of good schools and strong communities. At a time of troubling ethnic strife in many parts of the world, an effective American immigration policy can demonstrate to other countries that religious and ethnic diversity are compatible with national civic unity in a democratic and free society. [...] The recommendations in this report strongly affirm the value of a properly regulated immigration system. They seek to maximize the many positive opportunities that legal immigration presents to our nation. Programmatic Environmental Impact Statement For Northern Border Activities: Final Vol. 1
"U.S. Customs and Border Protection (CBP), a component of the Department of Homeland Security (DHS), proposes the use of a multilayered law enforcement approach to deploy border security program elements in the most effective combination to respond to any evolution of cross-border terrorist, criminal, and public safety threats along the northern border over the next five to seven years. Border security program elements consist of facilities; technologies for communication, detection, inspection, and surveillance; and land-based security infrastructure. These assets are used by agents, officers, specialists, and other personnel to pursue effective control of air, land, and sea borders between the United States and Canada. Under this proposal, CBP is evaluating alternative programmatic approaches that focus on augmenting particular elements for future responses to evolving threat s and changes in security or trade and travel facilitation priorities. [...] The purpose of the proposed action is to provide CBP with the flexibility to expand or alter its activities as needed to maintain effective control of the northern border in response to changes in priorities along that border. The proposed action is needed to better enable CBP agents and officers to operate in support of the CBP mission to safeguard the Nation. Improved border security will exist when CBP is able to stay abreast of current border activities; that is, to maintain situational awareness, determine the level of threat involved in given situations, and work in collaborative partnerships with local, state, and tribal law enforcement partners."

This report examines (1) the roles and responsibilities of the Board and the Police Chief and the comparability of the Board's scope to other law enforcement oversight entities; (2) the extent to which the Board's Manual incorporates leading practices for accountability, transparency, and external communication, and how the Board implements these practices; and (3) Congressional stakeholder perspectives on the Board's approaches and adjustments to enhance them. To complete this review, GAO analyzed relevant statutes and Board governing documents and operations. GAO also used internal control and corporate governance standards to articulate the key principles of accountability, transparency, and effective external communication; identified six leading practices that facilitate these principles; and analyzed the Board's Manual against each. GAO also interviewed Board members and staff, Congressional stakeholders, and experts selected for their knowledge of law enforcement oversight. GAO recommends that the Board revise its Manual to fully incorporate leading practices, including evaluating its performance, and engage with stakeholders and incorporate their views, as appropriate, on any changes.
