Final Exercise Report: Farley Nuclear Power Plant [August 21, 2002]
From the Executive Summary: "On August 21, 2002, the Federal Emergency Management Agency (FEMA) conducted a plume exposure pathway exercise in the emergency planning zone (EPZ) around the Farley Nuclear Power Plant. The purpose of the exercise was to assess the level of State and Local preparedness in responding to a radiological emergency. In this exercise the State of Alabama fully participated and the State of Georgia partially participated. The State of Florida is affected by the ingestion pathway and sent liaisons to Alabama's Forward Emergency Operations Center (FEOC) to monitor the situation and to provide information to the Florida State Emergency Operations Center (SEOC). This exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures. [...] This report contains the evaluation of the biennial exercise and the following out-of-sequence activities: Protective actions for schools, traffic and access control points (TCP), emergency worker decontamination, reception center-congregate care, forward command post (FCP), special populations and the distribution of potassium iodide (KI). State and local organizations, except where noted in this report, demonstrated knowledge of their emergency response plans and procedures and implemented them. No Deficiencies were identified. The one Area Requiring Corrective Action (ARCA) identified during this exercise concerned the provision of permanent-record dosimetry to emergency workers at a reception center and emergency worker decontamination station in Early County. This ARCA was resolved during the demonstration."

From the Executive Summary: "On November 15,2006, a Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway exercise was conducted for the 10-mile Emergency Planning Zone (EPZ) around the LaSalle County Station by the U.S. Department of Homeland Security (DHS), Chicago Field Office (CFO). The purpose of the exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS' policies and guidance concerning the exercise of State and local radiological emergency response plans (RERPs) and procedures. The most recent exercise at this site was conducted on March 10, 2004. [...] This Final Report contains the evaluation of the biennial exercise, and the evaluation of the following out-of-sequence activities: Illinois State Police (ISP) Dosimetry Control Officers Briefing, ISP Traffic and Access Control Point at the ISP District 17 Headquarters and Protective Actions for Schools (EV-2) Interview. The State and Local organizations except where noted in this report demonstrated knowledge of their emergency response plans and procedures and adequately implemented them[.]"

Exercise Report: Kewaunee Power Station [March 11, 2008]
From the Executive Summary: "On March 11, 2008, a Remedial Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise was conducted commensurate with the Deficiency and six associated Areas Requiring Corrective Action (ARCAs) concerning State Field Team operations that were assessed against the State of Wisconsin during the previous Kewaunee Power Station (KPS) exercise conducted on December 4, 2007. The remedial exercise was conducted for the 10-mile Emergency Planning Zone (EPZ) around the Kewaunee Power Station (KPS) by the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA), Region V. The purpose of the exercise was to assess the level of State preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMAs policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures. The most recent exercise at this site was conducted on December 4, 2007. [...] This Final Report contains the evaluation of the remedial exercise. Evaluated off-site response functions included the State Radiological Coordinator (SRC) Room, Field Operations Center/Mobile Communications Center (FOCIMCC) and State Field Teams. Although the SRC staff were not formally evaluated, as they had met all of the Criteria for which they were responsible during the December 4, 2007, biennial exercise, observations concerning their participation during the March 11, 2008, remedial exercise are included in this report in order to help clarify actions taken by the field operations staff at the FOCIMCC and by the State Field Teams."

This a combined final report of the Comanche Peak Nuclear Power Plant exercise conducted on August 29, 2007, retrieved individually from the U.S. Nuclear Regulatory Commission website. From the Executive Summary: "On August 29, 2007, a biennial Radiological Emergency Preparedness (REP) exercise was conducted in the Plume Exposure Pathway Emergency Planning Zone (EPZ) around the Comanche Peak Nuclear Power Plant (CPNPP), Glen Rose, Texas. Personnel from the U.S. Department of Homeland Security/FEMA (DHS/FEMA) Region VI, evaluated all activities. The purpose was to assess the level of preparedness of state and local responders to react to a simulated radiological emergency at the CPNPP. [...] The report includes the identification of any exercise issues and recommendations for corrective action where appropriate." The purpose of the exercise was to assess the level of preparedness of State and local jurisdictions in responding to a radiological emergency. This exercise was held in accordance with the DHS/FEMA policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures. [...] Except where noted in this report, the State and local organizations demonstrated knowledge of their emergency response plans and procedures and adequately implemented them. The exercise resulted in no Level 1 Findings, no Level 2 Findings, and no Plan Issues for the State of Ohio and the Counties of Ottawa, Lucas, and Sandusky. Final After Action Report: Grand Gulf Nuclear Station [October 21, 2015]
From the Executive Summary: "On October 21, 2015, the Department of Homeland Security (DHS), Federal Emergency Management Agency (FEMA), Region IV, Radiological Emergency Preparedness (REP) Program staff evaluated a hostile action based (HAB) plume exposure pathway exercise in the emergency planning zones (EPZ) for the Grand Gulf Nuclear Station (GGNS). The evaluation of out of sequence activities conducted during the weeks of September 28 through October 2, 2015 are included in this report. These out-of-sequence (OOS) activities included: a medical services drill, protective actions for reception/congregate care centers, traffic control points (TCP) and emergency worker (EW) and vehicle monitoring and decontamination (EWD). This exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERP) and procedures. The evaluation team conducted this exercise using Homeland Security Exercise and Evaluation Program (HSEEP) methodology. [...] The GGNS 10 mile EPZ includes the risk counties of Claiborne County, Mississippi and Tensas Parish, Louisiana. FEMA Region IV conducts the REP evaluation of Mississippi and Claiborne County as outlined in this After Action Report (AAR). FEMA Region VI is responsible for the REP evaluation of Louisiana and Tensas Parish and issues a separate AAR. Officials and representatives from the States of Mississippi and Louisiana along with those state's plume pathway counties and parish participated in the exercise."

After Action Report: Radiological Emergency Preparedness Exercise, Grand Gulf Nuclear Station [March 29, 2017]
From the Executive Summary: "On March 29, 2017, the U.S. Department of Homeland Security, Federal Emergency Management Agency Region IV, Radiological Emergency Preparedness Program staff evaluated a plume-exposure-pathway exercise for the 10-mile emergency planning zone of the Grand Gulf Nuclear Station. The evaluations of out-of-sequence activities conducted March 8 and the week of March 13 - 17, 2017, are also included in this report. The Grand Gulf Nuclear Station, operated by the Entergy Corporation is located in Claiborne County, 29 miles south of Vicksburg and seven miles north of Port Gibson. The emergency planning zone encompasses Claiborne County, Mississippi and Tensas Parish, Louisiana; it also encompasses a small-unpopulated portion of Warren County, Mississippi, which is managed by Claiborne County. [...] Officials and representatives from participating agencies and organizations demonstrated knowledge of their emergency response plans and procedures and successfully implemented them during the exercise. Out of sequence activities were also successfully demonstrated. [...] Highlights of the exercise included the State of Mississippi's emphasis on the Homeland Security Information Network, which provided a dedicated, teleconference-level, face-to-face information sharing platform for real-time situational awareness. This real-time information sharing greatly enhanced situational awareness for decision makers in making realistic response decisions. This highlight demonstrates the commitment of all of the jurisdictions involved to improve their preparedness. FEMA will provide an Improvement Plan to the State of Mississippi that details the strengths and areas for improvement observed during the exercise. The Improvement Plan is published under separate cover for official use only[.]"

Final Exercise Report: H. B. Robinson Steam Electric Plant [October 7, 2003]
From the Executive Summary: "On October 7, 2003, a full participation exercise was conducted in the plume exposure emergency planning zone (EPZ) around the H. B. Robinson Steam Electric Plant. The purpose of the exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was conducted in accordance with the Federal Emergency Management Agency's (FEMA) policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures. The previous exercise at this site was conducted on October 9, 2001. [...] This report contains the evaluation of the biennial exercise and the evaluation of a Medical Service Drill that was conducted on October 8, 2003. The State and local organizations demonstrated knowledge of their emergency response plans and procedures and the ability to implement them. No Deficiencies or Areas Requiring Corrective Action (ARCA) were identified. Although DHEC [South Carolina Department of Health and Environmental Control] dispatched a mobile laboratory to the Darlington National Guard Armory. The mobile laboratory served as an intermediate screening station for samples. This laboratory did not demonstrate the capabilities of a radiological laboratory. The capability will be demonstrated during the January 2004, Oconee Exercise. The ARCA concerning the direction and control of field teams identified during the V. C. Summer Exercise was corrected."

Final Exercise Report: H. B. Robinson Nuclear Station [December 7, 1999]
From the Executive Summary: "On December 7, 1999, a partial participation exercise was conducted in the plume exposure emergency planning zone (EPZ) around the H. B. Robinson Nuclear Station. The purpose of the exercise was to assess the level of state and local preparedness in responding to a radiological emergency. [...] The previous exercise at this site was an ingestion pathway exercise that was conducted on November 17-18, 1997. [...] This report contains the evaluation of the biennial exercise and the evaluation of the following out-of-sequence activities: protective actions for schools, traffic and access control points, lake clearing, emergency worker decontamination and reception and congregate care centers. The state and local organizations, except where noted in this report, demonstrated knowledge of their emergency response plans and procedures and the ability to implement them. Only one Area Requiring Corrective Action (ARCA) was identified during the exercise and one ARCA remains from the November 17-18, 1997 exercise, because field teams were not deployed during this exercise. Two ARCAs identified during the 1997 H. B. Robinson exercise were corrected during other South-Carolina exercises: one during the August 18, 1998, Oconee Nuclear Station exercise and the other during the July 21, 1999 V. C. Summer Nuclear Station exercise. Four other previous ARCAs were corrected as was one ARCA from the July 21, 1999 V. C. Summer Nuclear Station exercise."

From the Executive Summary: "On October 9, 2001, a full participation exercise was conducted in the plume exposure emergency planning zone (EPZ) around the H. B. Robinson Nuclear Station. The purpose of the exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was conducted in accordance with the Federal Emergency Management Agency's (FEMA) policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures. The previous exercise at this site was conducted on December 7, 1999. [...] This report contains the evaluation of the biennial exercise and the evaluation of a Medical Service Drill that was conducted on October 106. The State and local organizations demonstrated knowledge of their emergency response plans and procedures and the ability to implement them. No Deficiencies or Areas Requiring Corrective Action (ARCA) were identified. An ARCA (ARCA) concerning the notification of the radiological field monitoring teams to ingest potassium iodide, identified during the 2001 V. C. Summer exercise was corrected during this exercise. Especially noteworthy was the intensity of all of the participants during this exercise. The events of September 11, 2001 have caused State and local officials to view these exercises from a much different perception."

From the Executive Summary: "On October 5, 1999, an exercise was conducted in the plume exposure pathway emergency planning zone (EPZ) around the Kewaunee Nuclear Power Plant by the Federal Emergency Management Agency (FEMA), Region V. The purpose of the exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans (RERPs) and procedures. The most recent exercise at this site was conducted on September 23, 1997. [...] This report contains the final evaluation of the biennial exercise. State and local organizations, except where noted in the report, demonstrated knowledge of their emergency response plans and satisfactorily implemented them. There were no Deficiencies identified. There was initially one Area Requiring Corrective Action [ARCA] identified in both Kewaunee and Manitowoc Counties, as described in the Narrative Report provided to the State and Counties during the participants briefing on October 8, 1999. The ARCAs for both Counties were cleared after discussion and further review of the issues."

From the Executive Summary: "On March 29, 2017, a Plume Phase Radiological Emergency Preparedness (REP) exercise was conducted in the plume exposure pathway emergency planning zone (EPZ) around the Grand Gulf Nuclear Station (GGNS) located in Port Gibson, Claiborne County, Mississippi. Personnel from the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA), Region VI, evaluated the exercise. The purpose was to assess the level of preparedness of state and local responders to react to a simulated radiological emergency at GGNS. [...] The previous exercise at this site was conducted on October 21, 2015. The qualifying emergency preparedness exercise was conducted in November 1981. There have been 18 evaluated exercises, including the exercise on March 29, 2017, plus several drills conducted since that time. [...] This report contains the final written evaluation of the Plume Phase exercise. The state and local organizations, except where noted in this report, demonstrated knowledge of their emergency response plans and procedures and adequately implemented them. There was one corrected Level 2 Finding and one Plan Issue identified during this exercise."

After Action Report/Improvement Plan: Grand Gulf Nuclear Station [September 25, 2012]
From the Executive Summary: "On September 25, 2012 an out-of-sequence Medical Services drill was conducted for the Grand Gulf Nuclear Station (GGNS), located in Port Gibson, Claiborne County, Mississippi. Personnel from the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA) Region VI evaluated all activities. The purpose of the drill was to assess the level of preparedness of local responders to react to a simulated radiological emergency at GGNS. The previous plume exercise was conducted on November 1, 2011. Personnel from the State of Louisiana, Riverland Medical Center, Northeast Ambulance Service and Grand Gulf Nuclear Station participated in the drill. Cooperation and teamwork of all the participants was evident during the drill and DHS/FEMA wishes to acknowledge these efforts. This report contains the final evaluation of the out-of-sequence drill. The participants demonstrated knowledge of their emergency response plans and procedures. There were 3 Areas Requiring Corrective Action observed during this demonstration. Two of the Areas Requiring Corrective Action were corrected during the demonstration."

[...] The County and local organizations demonstrated knowledge of and adequately implemented organizational emergency response plans and procedures. From the Executive Summary: "On June 28, 2018, the U.S. Department of Homeland Security's (DRS) Federal Emergency Management Agency (FEMA), Region V, evaluated a Medical Services (MS-1) Drill at the Order of Saint Francis (OSF) St. Joseph Medical Center associated with the Clinton Power Station. The purpose of the MS-1 Drill was to assess the ability of off site agencies to respond to a medical emergency involving a potentially radiologically contaminated member of the public. The MS-1 Drill was held in accordance with DHS/FEMA policies and guidance concerning the exercise of State and local radiological emergency response plans (RERPs) and procedures. [...] The State and local organizations demonstrated knowledge of and adequately implemented organizational emergency response plans and procedures. There were no Level 1 Findings, Level 2 Findings or Plan Issues identified as a result of this drill and there were no previous Level 1 Findings, Level 2 Findings or Plan Issues to be corrected during this drill."

From the Executive Summary: "On June 17, 2016, the U.S. Department of Homeland Security's (DHS) Federal Emergency Management Agency (FEMA), Region V, evaluated a Medical Services (MS-1) Drill at the Order of Saint Francis (OSF) St. Joseph Medical Center associated with the Clinton Power Station. The purpose of the MS-1 Drill was to assess the ability of off site agencies to respond to a medical emergency involving a potentially radiologically contaminated member of the public. The MS-1 Drill was held in accordance with DHS/FEMA policies and guidance concerning the exercise of State and local radiological emergency response plans (RERPs) and procedures. [...] The State and local organizations demonstrated knowledge of and adequately implemented organizational emergency response plans and procedures. There were no Level 1 Findings, Level 2 Findings or Plan Issues identified as a result of this drill and there were no previous Level 1 Findings, Level 2 Finding or Plan Issues to be corrected during this drill."

Laboratory, Food Control Point, and Milk Sampling Drills: Washington Nuclear Project No. 2 (WNP-2) [August 12, 1999]
From the Executive Summary: "This report covers three out-of-sequence drills conducted on August 12, 1999, for Washington Nuclear Project No. 2 (WNP-2). A Laboratory Drill was conducted to demonstrate the capability of the Washington State Department of Health (WA DOH) Public Health Laboratories to analyze environmental samples for the presence of radiation, in response to a radiological incident at WNP-2. This was the third WA DOH Laboratory Drill conducted for WNP-2. The two previous drills were March 16, 1995, and April 29, 1992. A Milk Sampling Drill for WNP-2 [...] demonstrated the capability of the Washington State Department of Agriculture (WSDA) Food Safety Officer (FSO) to sample milk for radioiodine contamination at a dairy within the Emergency Planning Zone (EZP) of WNP-2. [...] [T]he Food Control Point (FCP) Drill for WNP-2 [...] demonstrated the capability and resources necessary to provide a method of interdicting potentially contaminated agricultural products to prevent them from entering intrastate and interstate commerce. [...] These drills were held in accordance with FEMA's regulation 44 CFR [Code of Federal Regulations] 350, and FEMA policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures. There were no Deficiencies identified or Areas Requiring Corrective Action (ARCAs) as a result of these out-of-sequence drills."

From the Executive Summary: "On July 20-22, 1999, a biennial Radiological Emergency Preparedness (REP) exercise was conducted in the Plume/Ingestion Exposure Pathway emergency planning zone (EPZ) around the Comanche Peak Steam Electric Station (CPSES), Glen Rose, Texas. The exercise was evaluated by the Federal Emergency Management Agency (FEMA) Region VI. The purpose was to assess the level of preparedness of State and local responders to react to a simulated radiological emergency at the CPSES. It was held in accordance with FEMA's policies and guidance concerning the implementation of State and local radiological emergency preparedness plans and procedures. [...] This report contains the written assessment of the biennial exercise including the identification of any exercise issues and recommendations for corrective action where appropriate. All State and local organizations, except where noted in this report, demonstrated an adequate knowledge of the emergency plans and procedures and properly implemented them. There were no Deficiencies and no Areas Requiring Corrective Action (ARCA) identified during this exercise."

The purpose of this exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures. The most recent exercise at this site was conducted on June 8, 2010. [...] Except where noted in this report, the State and local organizations demonstrated knowledge of and adequately implemented their emergency response plans and procedures. There were no Deficiencies identified for the State of Michigan or the Counties of Monroe or Wayne during this exercise. There was one Area Requiring Correction Action (ARCA) identified during this exercise for Monroe County.
