Foreign-Trade Zones: Board Should Document Consideration of All Required Criteria When Evaluating Applications, Report to Congressional Requesters
"GAO [Government Accountability Office] was asked to review the Board's evaluation processes. This report examines the extent to which the Board has established and followed procedures aligned with regulations for evaluating (1) notifications and (2) applications. GAO analyzed the Board's regulations and procedures and interviewed Commerce, Treasury, and U.S. Customs and Border Protection officials. GAO also analyzed a nongeneralizable sample of 59 of 293 notifications the Board evaluated from April 2012 through September 2017, which GAO selected to include a range of Board decisions and exclude pending decisions. GAO also analyzed all three applications the Board issued decisions on during that period."

"This guide is issued by the Office of the Under Secretary of Defense for Acquisition and Sustainment (OUSD(A&S)), the organization responsible for promulgation of policy for Other Transactions (OTs). This guide provides advice and lessons learned on the planning, publicizing, soliciting, evaluating, negotiation, award, and administration of OTs, to include all three types of OT agreements: Research, Prototype, and Production. While this document includes references to the controlling statutory and policy provisions for DoD OT authority, the document itself is not a formal policy document. Activities seeking to award OTs should consult with legal counsel for interpretation of statutory, regulatory, and formal policy requirements. If a strategy, practice, or procedure is in the best interest of the Government and is not prohibited by law or Executive Order, the Government team should assume it is permitted."

Electromagnetic Defense Task Force (EDTF): 2018 Report
"From 20-22 August 2018, Air University brought together leading subject matter experts from government, industry, laboratories, and academia to discuss vulnerabilities and threats, raise awareness, and explore mitigation strategies on an array of national security challenges in the EMS [electromagnetic spectrum]. The inaugural summit was attended by more than 135 military and civilian personnel representing more than 40 United States Department of Defense organizations, NATO, academia, and the private sector. During the summit, working groups focused on electromagnetic pulse (EMP), geomagnetic disturbance (GMD), lasers and optics, directed energy (DE), high-power microwaves (HPM), and EMS management. The summit was designed to challenge contemporary thinking and develop original thought about EMS and encourage actions to recover the technological initiative. Another priority was to immediately address the widening EMS threats to the United States and its allies. This report provides a summary of insights, conclusions, and recommendations developed during the inaugural summit"

Central America: USAID Assists Migrants Returning to Their Home Countries, but Effectiveness of Reintegration Efforts Remains to Be Determined, Report to Congressional Requesters
"In 2014, instability driven by insecurity, lack of economic opportunity, and weak governance led to a rapid increase of unaccompanied alien children (UAC) from El Salvador, Guatemala, and Honduras arriving at the U.S. border. In fiscal year 2017, the Department of Homeland Security reported (DHS) apprehending more than 200,000 nationals from these countries and removed nearly 75,000 nationals, including UAC, of these countries from the United States and returned them to their home countries. Current estimates also indicate nearly 350,000 individuals may need to be reintegrated to El Salvador and Honduras over the next few years when their Temporary Protected Status in the United States expires. GAO [Government Accountability Office] was asked to review U.S. efforts to support the reintegration of Central American migrants. This report describes (1) USAID [United States Agency for International Development] efforts to assist reception and reintegration of migrants from El Salvador, Guatemala, and Honduras into their home countries since fiscal year 2014; and (2) what is known about the effectiveness of these efforts. GAO reviewed agency program documents and funding data; interviewed officials from U.S. government agencies, IOM [International Organization For Migration], and host governments and beneficiaries; and conducted site visits in these countries. GAO is not making any recommendations in this report. USAID and IAF [Inter-American Foundation] provided formal comments, which are reproduced in this report, and all agencies provided technical comments, which were incorporated as appropriate."

Task Force on Survivable Logistics: Executive Summary
"The Defense Science Board (DSB) Task Force on Survivable Logistics was established to evaluate the current state of the U.S. military's Joint Logistics Enterprise (JLEnt), to assess high-end threats to the JLEnt from strategic competitors such as Russia and China, and to provide recommendations for securing and sustaining the JLEnt in an increasingly contested global security environment. Since the end of the Cold War, the United States has not fought an adversary capable of the catastrophic disruption of military supply chains and deployment of personnel and materiel. As a result, the JLEnt has suffered neglect and chronic underfunding relative to other DoD priorities. Simultaneously, the ability of strategic competitors to threaten the JLEnt has increased as they invest in anti-access/area denial (A2AD) capabilities and gray zone tactics. Upon completion of the study, the Task Force found that survivable logistics are essential for continued U.S. power projection and readiness against strategic competitors. However, DoD logistics capabilities require renewed attention to ensure they will be able to achieve mission success in a contested environment. Without a demonstrably resilient and survivable logistics capability, U.S. deterrence will suffer and the ability of the U.S. military to operate globally will be at stake."

Inland Waterways: Actions Needed to Increase Budget Transparency and Contracting Efficiency, Report to Congressional Requesters
"GAO [Government Accountability Office] was asked to review options to change the management of inland waterways. Among other things, this report assesses how the Corps [U.S. Army Corps of Engineers] allocates funds for operations and maintenance for the inland waterways, describes how the Corps funds construction projects, and assesses the effect of the current funding approach on projects' costs and schedules. GAO reviewed Corps documents and data; interviewed officials from Corps headquarters, six districts, and representatives of regional and national stakeholder groups--including commercial and recreational interests as well as contributors to relevant literature--selected to achieve a variety of viewpoints; and developed a simulation of the effect of various funding approaches on the total funding requirements and timelines for a set of hypothetical construction projects."

Defense Management: DOD Should Take Additional Actions to Enhance Corrosion Prevention and Mitigation Efforts, Report to the Committee on Armed Services, U.S. Senate
"A DOD-contracted study reported the cost impact of corrosion to DOD was$20.6 billion in fiscal year 2016. Corrosion can affect military readiness by taking critical weapon systems out of action and creating safety hazards. Corrosion can also lead facilities to experience structural failures and loss of capital investments. Senate Armed Services Committee Report 115-125, accompanying a bill for the Fiscal Year 2018 National Defense Authorization Act, included a provision for GAO [Government Accountability Office] to review aspects of DOD's corrosion prevention and mitigation efforts. In this report, among other things, GAO assesses the extent to which DOD has (1) consistently reported the funding levels needed to perform the Corrosion Executives' duties and (2) provided oversight of corrosion planning for major weapon system programs. GAO analyzed DOD guidance, funding (fiscal years 2010-2017) and corrosion planning information, and interviewed officials charged with overseeing DOD's corrosion efforts."

Arctic Planning: Navy Report to Congress Aligns with Current Assessments of Arctic Threat Levels and Capabilities Required to Execute DOD's Strategy, Report to Congressional Committees
"Public Law 115-91 required the Navy to report to Congress on the Navy's capabilities in the Arctic, including any capability gaps and requirements for ice-hardened vessels. It also included a provision for GAO [Government Accountability Office] to review the Navy's report. This report (1) assesses the extent to which the Navy's report aligns with current assessments of Arctic threat levels and capabilities required to execute DOD's 2016 Arctic Strategy and (2) describes any current requirements for ice-hardened vessels and DOD's approach for evaluating the capabilities needed as Arctic requirements evolve. GAO reviewed the Navy's report along with DOD's assessments of Arctic threats and naval capabilities. GAO also reviewed the 2016 DOD Arctic Strategy--the most current strategy, DOD and Department of State information on the freedom of navigation program as well as DOD's processes for developing capabilities and assessing Arctic capability gaps."

Global Development Lab: USAID Leverages External Contributions but Needs to Ensure Timely Data and Transparent Reporting, Report to Congressional Committees
"The Lab was created as a USAID [U.S. Agency for International Development] bureau in April 2014. The Lab was intended to institutionalize and improve USAID's ability to harness and leverage science, technology, innovation, and partnerships in addressing development issues and goals worldwide. The Lab supports projects and activities and announces, issues, and manages awards--or funding opportunities--for innovators to propose new ideas, approaches, and technologies. The Lab also incorporates external (i.e., non-USAID) contributions into its programming. Senate Report 114-290 included a provision for GAO [Government Accountability Office] to review the Lab. GAO's report examines, among other things, (1) the Lab's programs, funding, and staffing resources and (2) the extent to which the Lab has documented its oversight of awards with non-USAID contributions and clearly reported these contributions. GAO reviewed and analyzed agency documents and interviewed agency officials in Washington, D.C., and from six missions. GAO also analyzed selected Lab documentation for fiscal years 2014 through 2017."

Federal Real Property Asset Management: Agencies Could Benefit from Additional Information on Leading Practices, Report to Congressional Requesters
"GAO [Government Accountability Office] was asked to examine federal agencies' real property asset management practices and the applicability of ISO [International Organization for Standardization] 55000. This report discusses: (1) key characteristics of an effective asset management framework and how selected federal agencies' frameworks reflect these characteristics, and (2) whether government-wide asset management guidance and information reflect standards and key characteristics of an effective asset management framework, among other objectives. To conduct this work, GAO reviewed the ISO 55000 standards, relevant studies and literature, and interviewed 22 experts and 10 practitioners. GAO selected six federal agencies as case studies, including agencies with the largest real property portfolio and some agencies that were using the ISO 55000 standards. GAO reviewed documentation and interviewed officials from these six agencies, GSA [General Services Administration], and OMB [Office of Management and Budget]."

Defense Logistics: Actions Needed to Enhance the Security of High-Risk Ammunition at Storage Locations, Report to Congressional Committees
"DOD maintains stockpiles of ammunition for use during operations, including SRC [Security Risk Category] I ammunition. SRC I ammunition requires a high level of protection and security due to its ability to cause extreme damage or lethality. Senate Report 114-255 (2016), included a provision that GAO [Government Accountability Office] evaluate how DOD and the military services have established and maintained physical security measures at DOD locations with SRC I ammunition. GAO's report evaluates the extent to which (1) military service guidance is consistent with DOD's requirements for safeguarding SRC I ammunition and (2) the military services have identified and resolved physical security deficiencies at selected locations that store SRC I ammunition. GAO reviewed DOD guidance, visited selected military locations that were chosen based on size and variety of SRC I inventory, and interviewed officials. GAO also analyzed security inspection reports from 2014 to 2017."

Agent Orange: Actions Needed to Improve Accuracy and Communication of Information on Testing and Storage Locations, Report to Congressional Addressees
"The House report accompanying a bill for the National Defense Authorization Act for Fiscal Year 2018 included a provision that GAO [Government Accountability Office] review the government's handling of Agent Orange on Guam. This report examines (1) information the federal government has about the procurement, distribution, use, and disposition of Agent Orange; (2) DOD and VA [Veterans Affairs] efforts to make information about where Agent Orange and its components were tested and stored available; and (3) challenges associated with Agent Orange testing. GAO reviewed agency policies, documents, and available archival records that GAO identified; interviewed DOD, VA, and other agency officials; and met with a non-generalizable sample of 38 veterans and a veterans service organization."

Tribal Broadband: FCC Should Undertake Efforts to Better Promote Tribal Access to Spectrum, Report to Congressional Requesters
"This report examines (1) tribal entities'ability to obtain and access spectrum to provide broadband services and the reported barriers that may exist, and (2) the extent to which FCC [Federal Communications Commission]promotes and supports tribal efforts to obtain and access spectrum. GAO [Government Accountability] interviewed 16 tribal entities that were using wireless technologies. Selected entities varied geographically, among other characteristics. GAO analyzed FCC's license and auction data as of September 2018, reviewed FCC's rulemakings on spectrum for broadband services, and interviewed other tribal and industry stakeholders and FCC officials. The information presented is not generalizable to all tribes or industry participants."

Illegal Marijuana: Opportunities Exist to Improve Oversight of State and Local Eradication Efforts, Report to Congressional Requesters
"Marijuana is generally illegal under federal law. Nonetheless, an increasing number of states have legalized medical or recreational marijuana under state law. However, in these states, some marijuana-related activity may still be illegal under state law. Since 1981, DEA's [Drug Enforcement Administration] DCE/SP [Domestic Cannabis Eradication/Suppression Program] has provided financial support to participating state and local agencies for their efforts to eradicate illegal marijuana. GAO [Government Accountability Office] was asked to review DEA's DCE/SP. This report examines (1) DCE/SP funding and expenditures in recent years, (2) how DEA ensures that participating agencies expend funds in accordance with program requirements, and (3) how DEA uses performance assessment to help manage DCE/SP. GAO analyzed DCE/SP guidance, and expenditure and performance information from 2015 through fiscal year 2017, and evaluated DEA's oversight and performance management efforts against internal control standards. GAO also interviewed officials from DEA, the U.S. Forest Service, and participating agencies in six states, which GAO selected to include varying levels of DCE/SP funding and numbers of marijuana grow sites eradicated in recent years."

Depot Maintenance: DOD Has Improved the Completeness of Its Biennial Core Report, Report to Congressional Committees
"DOD uses both military depots and contractors to maintain its complex weapon systems and equipment. Recognizing the depots' key role and the risk of over reliance on contractors, section 2464 of title 10 of the U.S. Code requires DOD to maintain a core logistics capability that is government-owned and operated, involving a combination of personnel, facilities, equipment, processes, and technology. Section 2464 requires DOD to provide a Biennial Core Report to Congress that addresses 10 reporting elements, including information on its core capability requirements and projected workload for the next fiscal year. Section 2464 includes a provision that GAO [Government Accountability Office] review DOD's Biennial Core Reports for compliance and completeness. In reviewing the 2018 Biennial Core Report, GAO assessed the extent to which DOD's report (1) addressed the 10 reporting elements required by section 2464(d), and (2) is complete. GAO reviewed and analyzed relevant legislation, DOD guidance, and the 2018 Biennial Core Report, and met with DOD and military service officials to discuss the processes used to develop the information in DOD's 2018 Biennial Core Report."

Sheltering in Danger: How Poor Emergency Planning and Response Put Nursing Home Residents at Risk During Hurricanes Harvey and Irma
"Every time a hurricane strikes the United States, hundreds of thousands, even millions, of people face a difficult decision: heed the warnings of local officials and evacuate the area, or ride out the storm by 'sheltering in place.' This decision can be a matter of life or death, especially for people living in low-lying areas vulnerable to flooding and storm surges. For nursing homes and assisted living facilities entrusted to take care of frail residents with complex medical needs, the decision to evacuate or shelter-in-place takes on even greater weight. Administrators of these facilities are not only considering their own safety, but that of residents unable to fend for themselves, and the staff who care for them. Many nursing homes and assisted living facilities chose to shelter-in-place when hurricanes Harvey and Irma struck Texas and Florida, respectively, late in the summer of 2017. While most of these facilities weathered the storms without incident, the exceptions were glaring and tragic. [...] This report examines the decisions that were made before, during and after the 2017 storms, as well as gaps in federal regulations currently on the books. It makes recommendations on how to avoid these types of tragedies in the future."

Winter Residential Building Fires (2014-2016)
"Each year, from 2014 to 2016, fire departments responded to an estimated 108,200 fires in residential buildings during the months of January, February and March across the nation. These fires resulted in an estimated annual average of 980 deaths, 3,575 injuries, and $1.9 billion in property loss. The residential building portion of the fire problem is of great national importance as it accounts for the vast majority of civilian casualties, and winter residential building fires account for a considerable portion of that."

NIH Medical Research Program Directed Against Chemical Threats: 2017 Report on Research Progress and Future Directions
"The general health threat posed by chemical agents has created numerous challenges for many departments and agencies across the U.S. government. Unlike infectious diseases and radiological and nuclear exposures, where a latent period may allow for prophylactic measures to be instituted prior to the manifestation of clinical symptoms, injuries from chemical exposures can occur rapidly with immediate casualties and fatalities. Many of these toxic chemicals or their precursors may be easily procured or manufactured undetected. Some of these chemicals do not even require specialized scientific expertise or knowledge for their synthesis and use. In addition, the sheer number and variety of chemicals that pose a health risk to civilians is especially daunting. [...] It is impractical to develop specific medical countermeasures (MCMs) against every dangerous chemical available, so a prioritization system is necessary to focus MCM research and development efforts. As such, the U.S. Department of Homeland Security (DHS) Chemical Security Analysis Center (CSAC) was tasked with analyzing and prioritizing chemicals that have been deemed to be threats to civilians. In support of this prioritization effort, CSAC developed the Chemical Terrorism Risk Assessment (CTRA) and Chemical Infrastructure Risk Assessment (CIRA) programs, which use probabilistic risk assessments to quantify and prioritize chemical risk. [...] The purpose of this report is to provide an update on the progress of NIH's [National Institutes of Health] chemical MCM research and early development program between fiscal years 2011 and 2017 and offer insight into future directions."

Navy Readiness: Actions Needed to Address Costly Maintenance Delays Facing the Attack Submarine Fleet, Report to the Subcommittee on Readiness, Committee on Armed Services, House of Representatives
"GAO [Government Accountability Office] was asked to review the readiness of the Navy's attack submarine force. This report discusses the extent to which the Navy (1) has experienced maintenance delays in its attack submarine fleet and costs associated with any delays; and (2) has addressed any challenges and developed mitigation plans for any maintenance delays. GAO analyzed readiness information from fiscal years 2008-2018, operating and support costs, maintenance performance, and other data; visited attack submarines and squadrons; and interviewed public and private shipyard and fleet officials. This is a public version of a classified report issued in October 2018. Information the Department of Defense deemed classified or sensitive, such as attack submarine force structure requirements and detailed data on attack submarine maintenance delays, has been omitted."

Centers for Disease Control and Prevention Has Controls and Strategies to Mitigate Hurricane Preparedness and Response Risk
"Three major hurricanes hit the United States between August and September of 2017. On September 8, 2017, the President signed the first Federal Disaster Assistance package of the 2017 hurricane season following Hurricane Harvey. On February 9, 2018, the President signed into law the Bipartisan Budget Act of 2018 (P.L. No. 115-123), which provided disaster relief totaling approximately $89.3 billion. HHS [United States Department of Health and Human Services] received $5.97 billion ($1.07 billion for discretionary programs), of which the Centers for Disease Control and Prevention (CDC) received $200 million. Our objectives were to identify CDC's potential risks for preparing for and responding to hurricanes and other natural disasters and to determine whether CDC has designed and implemented controls and strategies to mitigate these potential risks."

Emergency Prescription Assistance Program (EPAP) Hurricanes Irma & Maria- USVI Data Fact Sheet
"The Emergency Prescription Assistance Program (EPAP) is funded by the Stafford Act and is designed to help disaster survivors access prescription medicines. EPAP can also be activated by the Public Health Service Act under the authority of the National Disaster Medical System (NDMS). The program utilizes normal business operations (e.g., electronic prescription claims processing, utilization of the normal pharmaceutical supply chain for distribution and dispensing) to pay for prescription medications for eligible persons. This fact sheet provides a summary of the EPAP data collected and analyzed for the US Virgin Islands (USVI) only following Hurricanes Irma and Maria from November 2, 2017 to September 15, 2018."

Defense Nuclear Enterprise: DOD Continues to Address Challenges but Needs to Better Define Roles and Responsibilities and Approaches to Collaboration, Report to Congressional Committees
"The National Defense Authorization Act for Fiscal Year 2017 includes a provision for GAO [Government Accountability Office] to review DOD's processes for addressing these recommendations. This report addresses the extent to which DOD and the military services have (1)made progress in the implementation, tracking, and evaluation--including identifying and documenting risk--of the recommendations of the 2014 nuclear enterprise reviews and the 2015 NC3 report and (2) improved oversight of the defense nuclear enterprise and managed roles, responsibilities, and collaboration among various organizations. GAO reviewed relevant documents and interviewed agency officials from DOD and the military services."
