  From the opening statement of Jeff Bingaman: "Today we want to have testimony on two bills that seek to improve our emergency petroleum reserve policy. In the last few years we've experienced several oil market disruptions that resulted from hurricanes taking operations in the Gulf of Mexico region offline. Most recently in September 2008, Hurricanes Gustav and Ike caused a disruption that left many gas stations throughout the Southeast without sufficient gasoline and diesel for weeks following the storms. It strikes me that we can and should do better than that. We expect to be facing a world with more hurricanes and more extreme weather situations in the future. We need to decide either to learn to live with temporary regional gasoline and diesel outages or we need to be prepared to deal with the aftermath of these storms in a better way than we have in the past and prevent these kinds of shortages. I believe we should take this latter approach. Preventing these kinds of hurricane-related shortages is an achievable goal. We've learned in this country that when oil supplies are disrupted, economic hardships follow. This is not simply about inconvenience to those regions whose transport might be temporarily constrained. It's also about making sure that there is sufficient fuel for emergency services and about minimizing longer lasting economic damage that results from price spikes and disrupted commerce." • Development of Regression Equations for the Estimation of Flood Flows at Ungaged Streams in Pennsylvania

  From the Purpose and Scope: "This report presents the estimated magnitude of flood flows associated with the 50-, 20-, 10-, 4-, 2-, 1-, 0.5-, and 0.2-percent AEPs for 356 streamflow gaging stations (referred to hereafter as streamgages) across Pennsylvania and surrounding states not subject to substantial flow regulation, diversion, or mining activity. [...] The streamgages included in the study had a minimum of 10 years of data through water year 2015. The report also documents a regional skew analysis, trends in annual peak streamflow data, and development of regional flood-flow regression equations from the relation of flood-flow and basin characteristic data. The resultant regression equations allow for the computation of flood flows at ungaged basins across Pennsylvania by means of select basin characteristics that are significantly correlated with streamflow. Estimates of floodflow magnitude and frequency for streamgages substantially affected by upstream regulation are also presented. The limitations of the study and uncertainties of the flood-flow estimates are also discussed."

Among those are to provide for the development of one authorized geologic repository for the permanent disposal of waste. The Act also authorizes one commercial spent nuclear fuel interim storage facility under certain conditions. The Department's fiscal year 2010 budget request announced the administration's intended termination of the Yucca Mountain Repository Project and includes the funding needed to explore alternatives for nuclear waste disposal and to continue participation in the Nuclear Regulatory Commission license application process. However, the Department remains committed to meeting its obligations for managing and ultimately disposing of spent nuclear fuel and high level waste. To that end, the Secretary of Energy is convening a blue ribbon panel of experts to evaluate alternative approaches for meeting the federal government's responsibility." • Pre-hospital Emergency Medical Service Calls Where Patients Are Not Transported

  "Some patients who are not transported by ambulance need further care. The purpose was to identify reasons for non-transportation of patients. Descriptive research answered questions: (1) What standards of care exist to guide paramedics who perform field assessments to determine patients' needs for emergency transportation to the hospital? (2) How accurate are paramedic field assessments in determining patients' needs for definitive care offered in hospitals? (3) What types of calls have an increased incidence of patients who are not transported, but who later are determined to need additional medical care? (4) What are reasons Boynton Beach Fire Rescue paramedics cite for non-transportation of patients? (5) How does the quality assurance program at Boynton Beach Fire Rescue address occurrences of non-transportation of patients? Results of literature review, records review and data collection provided information establishing a standard of care and revealed paramedic accuracy issues related to diabetic patients and fall victims who frequently need additional medical care. Recommendations include: revise the quality assurance process; protocol changes; revise the refusal process; revise the current feedback mechanism; create a patient safety advocate position; eliminate terms that lead to under triage; and monitor results annually."

  The "Homeland Security Information Sharing Act [107-534, Part 1]" is a 69-page House of Representatives report about the need for legislation with regards to the sharing of information. This report references the USA PATRIOT Act and H.R. 4598: The USA PATRIOT Act allows the facilitation of information between Federal law enforcement officials and the intelligence community. Thus, the very purpose of that bill was to improve information sharing for the law enforcement and intelligence communities to combat terrorism and terrorist-related crimes. H.R. 4598 "Homeland Security Information Act," requires the President to create procedures to strip out classified information so that State and local officials may receive the information without clearances. The bill also removes the barriers for State and local officials to share law enforcement and intelligence information with Federal officials. H.R. 4598 contains no intergovernmental or private-sector mandates as defined in the Unfunded Mandates Reform Act and would impose no costs on State, local, or tribal governments. The bill would benefit State and local governments by allowing these levels of government better access to homeland security information. H.R. 4598 would direct the President to establish guidelines for Federal agencies to share homeland security information with State and local personnel.

  "In response to the recent serious recession, Congress enacted the Recovery Act to promote economic recovery, make investments, and minimize or avoid reductions in state and local government services. Approximately $219 billion was distributed as grants for use in states and localities, making grants a major component of the act. These grants covered a broad range of areas including education, transportation, energy, infrastructure, the environment, health care, and housing. GAO [Government Accountability Office] was asked to examine grant management lessons learned resulting from the Recovery Act. This report examines federal, state, and local experiences with implementing grants funded by the Recovery Act by identifying examples of good practices employed and challenges faced in meeting the act's accountability and transparency requirements. GAO reviewed relevant documents including OMB and Recovery Board guidance, relevant literature, and previous reports by GAO, federal inspectors general, and others. GAO also interviewed officials from OMB, the Recovery Board, four federal agencies, three state governments, and two local governments, among others. This report also draws on GAO's past bimonthly reviews of selected states' and localities' use of Recovery funds."

  "The Unified Command Plan (UCP) assigns the missions of Information Operations (IO) and Global Command, Control, Communications, Computers, Intelligence, Surveillance, and Reconnaissance (C4ISR) to Commander, US Strategic Command (CDRUSSTRATCOM). Included in these missions is the responsibility to operate and defend the Global Information Grid (GIG). NetOps [Network Operations] is the operational construct that the CDRUSSTRATCOM will use to operate and defend the GIG. The goal of NetOps is to provide assured and timely net-centric services across strategic, operational and tactical boundaries in support of DOD's [Department of Defense] full spectrum of war fighting, intelligence and business missions. • Emergence of Organizational Fit: Applying Configuration Theory to the Snohomish County (WA) Emergency Operations Center

  From the thesis abstract: "An emergency operations center (EOC) plays a critical role during a community's response to a disaster. Yet, little research exists that assesses the impact of the organization's structure on an EOC staff's performance. This thesis explores how configuration theory, which emphasizes the need for an organization's structure to fit the situation, can help emergency managers organize an EOC's staff to improve its performance. Specifically, it uses configuration theory to examine the Snohomish County (WA) EOC's response to the State Route 530 flooding and mudslides incident. From that case study, three workgroups were selected, and members of each workgroup were interviewed. The results were analyzed twice, first by distinct workgroup and then cumulatively. Among the findings is the diversity of influences on the task environments of different workgroups responding to the same incident; the findings also reveal that the groups implemented aspects of more than one structure. Based on these analyses, recommendations are made to update EOC doctrine and training to focus on situational factors and flexible configurations, including hybrid configurations. And while the setting for this research is a local EOC, its outcomes reinforce the applicability of configuration theory to any organization responding to a crisis."

From the Opening Statement of Chairman Shimkus: "Today, the subcommittee will be taking a look at the harmful algal blooms in drinking water, including the source water used for drinking. I thank Representative Latta for his efforts on this issue and for bringing it to the subcommittee's attention. He has delved into the minutia of this issue in search of useful and longterm solutions to this problem. Having sat in some of the meetings Mr. Latta has been having on this issue, I realize what a complex and widespread issue this is, but one which only gained national attention a few months ago. Some folks may be tempted to think there are easy solutions to this problem, but I caution jumping to simple or sweeping conclusions. There is no single 'smoking gun' that leads to algae-based toxins in drinking water, I believe we will hear our witnesses say there are still plenty of things we don't know about this subject."   "The world cruise of the Great White Fleet had dual origins, a diplomatic crisis with Japan and a need to test new U.S. battleships. On June 27, 1907, at the height of a crisis in Japanese-American relations, President Theodore Roosevelt decided to send the Atlantic battleship fleet to the Pacific in the fall. The 1904-5 Russo-Japanese War had demonstrated the military and naval might of Japan, while the decisive Russian defeat at Tsushima impressed on the public's consciousness that a battle fleet, after steaming great distances, could arrive at its destination in no condition to fight. With nearly all U.S. battleships in the Atlantic Fleet, the U.S. naval force in the Pacific was no match for the Japanese Imperial Navy. Aware of these conditions, West Coast residents of the United States felt vulnerable to attack."

• Firefighter Fatalities in the United States in 2011

  "For 35 years, the U.S. Fire Administration (USFA) has tracked the number of firefighter fatalities and con-ducted an annual analysis. Through the collection of information on the causes of firefighter deaths, USFA is able to focus on specific problems and direct efforts toward finding solutions to reduce the number of fire-fighter fatalities in the future. This information is also used to measure the effectiveness of current programs directed toward firefighter health and safety. Several programs have been funded by USFA in response to this annual report. For example, USFA has sponsored significant work in the areas of general emergency vehicle operations safety, fire department tanker/tender operations safety, firefighter incident scene rehabilitation, and roadside incident safety. The data developed for this report are also widely used in other firefighter fatality prevention efforts. In addition to the analysis, USFA, working in partnership with the National Fallen Firefighters Foundation (NFFF), develops a list of all onduty firefighter fatalities and associated documentation each year. If certain criteria are met, the fallen firefighter's next of kin, as well as members of the individual's fire department, are invited to the annual National Fallen Firefighters Memorial Weekend Service. The service is held at the National Emergency Training Center (NETC) in Emmitsburg, MD, during Fire Prevention Week in October of each year. [...] The specific objective of this study is to identify all onduty firefighter fatalities that occurred in the United States and its protectorates in 2011 and to analyze the circumstances surrounding each occurrence. The study is intended to help identify approaches that could reduce the number of firefighter deaths in future years."

From the opening statement of Benjamin L. Cardin: "Let me start by saying the United States has as much natural gas as Saudi Arabia has oil. According to Penn State, the Marcellus Shale which runs from Central New York State to West Virginia may be the second largest natural gas field in the world. We have enormous reserves that can help America meet its energy needs and do so in a way that produces far less pollution than coal, help the United States on its path to energy independence, and improve our national security. High volume horizontal hydraulic fracturing, or fracking, is now being used to extract natural gas from shale formations in thousands of new wells. In Pennsylvania, more than 2,700 Marcellus wells were drilled from 2006 to March 10 of this year. A study last year estimated that Marcellus drilling would create or support more than 100,000 jobs in 2011 plus billions of dollars in economic value for the State. The natural gas industry is booming, but it may be in jeopardy. New York has imposed a moratorium on fracking operations. New Jersey is considering a ban on the practice. The city of Pittsburgh has enacted a ban on fracking operations within the city limits. Tiny Mountain Lake Park, MD adopted an ordinance making the drilling of natural gas illegal within the town limits." From the opening statement of Mike Rogers: "Let me start by saying that giving up on TSA without having something better to fill its place is not an option. For all its faults, the fundamental reason TSA was set up after 9/11 was to deploy enhanced security measures to prevent another attack on aviation. That security mission is just as important today, if not more important, than it was 11 years ago. Having said that, letting TSA carry on the way it has for the last 11 years is equally not an option. TSA's poor conduct is sending a strong message to the American taxpayer. The message is TSA doesn't care or doesn't know how to best serve and protect the traveling public.   "Its subject is one that has been barely tackled anywhere else: What gives modern intelligence practitioners and their organizations their claims to professionalism, and what actually does this professionalism entail? Intelligence as an activity is partly a matter of special skills -- agent recruitment and running; cipher-breaking and other SIGINT techniques; imagery interpretation; and all the others -- but these merge into its broader professional canons for handling indications and evidence, drawing conclusions, and presenting these to decision-takers in ways that command attention while incorporating careful judgments of certainty and uncertainty. The intelligence professional is close to the policy adviser, but still keeps a certain distance from him. Intelligence's concern is with understanding and evaluating 'them,' rather different from the policy adviser's roots in 'us' and in what 'our' decisions should be. This book examines ways in which intelligence develops its characteristic standards of accuracy and duty. It considers the effects of formal legal codes and democratic oversight, but a principal conclusion emerging from it is the importance of professional training. Its implicit sub-text is indeed that standards of intelligence analysis and integrity should be properly taught, and not just caught by osmosis from one's seniors. At least for this British reader, the amount of formal analytic training in the relatively modest national intelligence systems described here is surprising and thought-provoking, perhaps salutary."

  "This Emergency Responder Field Operating Guide (ERFOG) is designed to assist emergency response personnel in the use of the National Incident Management System's (NIMS) Incident Command System (ICS) during incident operations. This Guide is intended for use when implementing ICS in response to an incident/event, regardless of type, size, or location. It is not a policy document and does not replace Emergency Operations Plans (EOPs), laws, regulations, and ordinances. Rather, this document provides guidance for assigned incident/event personnel. Chapters 2-12 of this document provide information applicable to all disciplines when implementing ICS. For example, the duties and responsibilities of the Planning Section Chief (PSC) are found in Chapter 9; the PSC's job description under ICS does not change from one type of incident to another. Each chapter lists specific job descriptions that have proven valuable in past response operations. Chapter 13 provides the incident commander and responder with additional resources including example decisions and objectives, an example staffing matrix and a list of federal resources."

  From the Executive Summary: "Organizations have information technology (IT) plans in place, such as contingency and computer security incident response plans, so that they can respond to and manage adverse situations involving IT. These plans should be maintained in a state of readiness, which should include having personnel trained to fulfill their roles and responsibilities within a plan, having plans exercised to validate their content, and having systems and system components tested to ensure their operability in an operational environment specified in a plan. These three types of events can be carried out efficiently and effectively through the development and implementation of a test, training, and exercise (TT&E) program. Organizations should consider having such a program in place because tests, training, and exercises are so closely related. For example, exercises and tests offer different ways of identifying deficiencies in IT plans, procedures, and training. This document provides guidance on designing, developing, conducting, and evaluating TT&E events so that organizations can improve their ability to prepare for, respond to, manage, and recover from adverse events that may affect their missions."

We must train the way we intend to fight. Our efforts must also ensure that resources are efficiently applied to develop and maintain an integrated and flexible joint force. As a goal, combatant commands and the Services should continue to develop complementary methodologies to better allocate joint training resources, thereby increasing jointness by improving the quality of joint training. Commanders at all echelons should honestly and aggressively define and assess their joint training requirements. The specific objective is to develop a joint training and exercise program that bolsters combatant commanders ability to execute the National Military Strategy (NMS) while simultaneously maintaining readiness as a prerequisite to deterring aggression and responding to crisis. The desired end state is the improved readiness of joint forces, a training and exercise strategy better aligned with the NMS, improved interoperability, and a more stable process for optimizing the application of scarce Service resources."

  This hearing discusses the cleanup and the reopening of the U.S. Postal Service's Joseph Curseen Jr. and Thomas Morris Jr. Processing and Distribution Center on Brentwood Road in Washington, DC. "We all remember the terrible events of October 2001, when the dedicated postal employees of the Brentwood Processing Center joined the front lines in the war on terrorism. Tragically, two workers, Joseph Curseen and Thomas Morris, lost their lives to anthrax, which passed through the building in letters addressed to Senators Daschle and Leahy. Two other postal workers were infected and the building has remained closed to this day. In July 2001, the Subcommittee on the District of Columbia held a hearing on the Postal Service's plan to clean and reopen the facility, an effort which at that point was just beginning. Now, with the cleanup complete and reopening scheduled for next month, we're following up on that hearing."   From the thesis abstract: "This thesis identified a need for improvement in first-responder crisis decision making. The primary research question was, would mindfulness training be worthwhile as a means to enhance first-responder crisis decision making? Qualitative research methods (primarily a thematic analysis of the literature) were utilized to explore the areas of first responder operations, crisis decision making, and mindfulness training to identify key categories in the data relevant to the primary and sub-research questions. Evidence uncovered during this research supports a conclusion that mindfulness training may be one way in which first responders can improve upon their capacity to make effective decisions during a crisis. It was demonstrated that these improvements to crisis decision making resultant from mindfulness training could very well culminate in significant improvement of outcomes during future emergencies and disasters at which mindful first responders are present. Despite significant implementation challenges that were identified, systematic analysis of the literature revealed sufficient evidence to warrant serious consideration for further study and application of this theory in the field, including potential implementation of discipline-specific and culturally relevant mindfulness training systems within first responder organizations."

  "This 'Protection of Civilians (PoC) Military Reference Guide' grew out of conversations and collaborations with a number of individuals and institutions working to prevent and respond to violence against civilians. Significant among these was a 2009 workshop convened by the Stimson Center at the UK Defence Academy in Shrivenham which engaged experts and doctrine writers alongside military and civilian leaders with experience in protection crises. It included a two-day simulation exercise involving escalating violence against civilians and challenged workshop participants to propose and evaluate courses of action to protect civilians. The workshop was organized by Alison Giffen and Max Kelly to capture insights that could be distilled into guidance for future missions mandated to protect. The project resulted in three products authored by Max Kelly and/or Alison Giffen: 'Addressing the Doctrinal Deficit: Developing Guidance to Prevent and Respond to Widespread or Systematic Attacks Against Civilians'; 'Protecting Civilians: Proposed Principles for Military Operations'; and 'Military Planning to Protect Civilians: Proposed Guidance for United Nations Peacekeeping Operations'. These publications detailed why doctrine on the protection of civilians is needed, outlined a strategic framework for the doctrine and proposed seven doctrinal principles for military operations facing protection challenges. Although the Reference Guide's content has evolved beyond Stimson's work and includes the input and expertise of many, Stimson's 'Addressing the Doctrinal Deficit' initiative and publications served as the point of departure for this Guide."
